- •(1)Patients who are hospitalized using a diabetes clinical path (guidance content and examinations are scheduled daily within 2 weeks) between the date of study implementation permission and December 31, 2022.
- •(2) Patients who consented to the study
- •(3)Patients scheduled to go to the outpatient department of diabetes, endocrinology and metabolism at the Nihon Keizai University Hospital after discharge.
- •(4) It is possible to communicate through words and exchange conversations with researchers.
排除标准
- •(1) Not an adult
- •(2) Diagnosed with dementia
- •(3) Impairment in taking self-management behavior (physical/visual/cognitive function)
- •(4) Diagnosed with stage IV nephropathy or later
