- •BMI over 25
- •English speaking and literate
- •Aged over 18 years
- •Has visited the GP at least 3 times in the last 2 years
- •Score of 6 or more on the AUSDRISK tool (Australian Type 2 Diabetes Risk Assessment Tool - validated tool for future risk of developing diabetes)
排除标准
- •BMI over 40
- •Uncontrolled medical or mental health condition
- •History of eating disorder
- •Breastfeeding
- •Age over 65 years
- •Taking weight loss medications
- •Past or scheduled bariatric surgery
- •Current history of diabetes, congestive heart failure, chronic renal failure, chronic obstructive pulmonary disease
- •Significant immobility
