- •The patient:
- •- stroke is the main cause of his dependency and has occurred longer than 3 months ago.
- •- lives in a nursing home in a 'somatic unit' (a unit for residents who have mainly physical handicaps);The informal caregiver:
- •- is the person to be contacted or representative of the patient.
- •- is closely in touch with the care for the patient.;The professional:
- •- is responsible for the daily care of the patient
排除标准
- •The patient:
- •- has severe communication deficits
- •- can not be loaded due to severe depression or bad physical condition
- •- is not able to give informed consent
