Effects of Gastrocnemius Training using Neuromuscular Electrical Stimulation on Foot Morphology, Balance and Gait in Patients with Stroke

First, changes in foot morphology parameters were as follows: ? In intra-group comparison of 3D foot scanning results, the experimental group showed significant changes in longitudinal arch angle (LAA) (p<.05), medial longitudinal arch angle (MLAA) (p<.01), transverse arch angle (TAA) (p<.01), rearfoot angle (p<.05), foot length (p<.05), foot width (p<.05), and arch height index (AHI) (p<.01) of the paretic side and in TAA (p<.05) and AHI (p<.05) of the non-paretic side. The control group showed significant changes in TAA (p<.05) and foot width (p<.05) of the paretic side and TAA (p<.05) and AHI (p<.05) of the non-paretic side. Inter-group comparison showed significant differences in MLAA (p<.05) and rearfoot angle (p<.05) of the paretic side. ? In intra-group comparison of knee-to-wall test, the experimental group showed significant differences in the results before and after the intervention (p<.05). Similarly, inter-group comparison also showed significant differences in the knee-to-wall test results between the control and experimental groups (p<.05). ? In intra-group comparison of concentric muscle activity, the experimental group showed significant differences in tibialis anterior muscle (p<.05), medial gastrocnemius muscle (MG) (p<.01), and lateral gastrocnemius muscle (LG) (p<.05) of the paretic side and in MG (p<.05), LG (p<.05), soleus muscle (p<.05) of the non-paretic side, and soleus muscle symmetry index (p<.05). The control group showed significant changes in MG (p<.05) of the paretic side and in MG (p<.05) and LG (p<.05) of the non-paretic side. Inter-group comparison showed a significant difference in the MG (p<.01) of the paretic side between the control and experimental groups. ? In intra-group comparison of eccentric muscle activity, the experimental group showed significant differences in MG (p<.01) and LG (p<.01) of the paretic side and in MG (p<.01), LG (p<.01) of the non-paretic side, and LG symmetry index (p<.01). The control group showed significant differences in MG of both the paretic side (p<.05) and non-paretic side (p<.05). In inter-group comparison, the two groups showed significant differences in MG (p<.05) and LG (p<.01) of the paretic side. Second, changes in balance parameters were as follows: ? In intra-group comparison of foot pressure assessment, the experimental group showed significant differences in foot weight-bearing area (p<.05) of the paretic side and foot weight-bearing area symmetry (p<.05). The control group showed a significant difference in only foot weight-bearing area symmetry (p<.05). Inter-group comparison showed no significant difference between the two groups (p>.05). ? In intra-group comparison of functional reach test, significant differences were noted in the test results in both the control and experimental groups (p<.05). However, no differences were noted between the two groups (p>.05). ? Intra-group comparison of Berg balance scale showed significant differences in both the control and experimental groups (p<.01). Similarly, inter-group comparison showed significant differences between the two groups (p<.01). Third, changes in gait parameters were as follows: In intra-group comparison, the experimental group showed significant differences in gait speed (p<.01), cadence (p<.01), stride length (p<.01), step length of the paretic side (p<.05), and step length of the non-paretic side (p<.01) and step length symmetry index (p<.05). The control group showed significant differences in gait speed (p<.05), cadence (p<.01), stride length (p<.01), and step length of the non-paretic side (p<.05). Inter-group comparison showed significant differences in stride length (p<.01) and step length of the non-paretic side (p<.05) between the two groups.

入选标准

  • First, it has been at least 6 months since the stroke was diagnosed.
  • Second, a score of 24 or more on the Korean version of the Mini-Mental State Examination.
  • Third, those who can walk independently for 10 meters or more
  • Fourth, those with limited passive range of motion due to ankle stiffness
  • Fifth, no joint contractures, pain, or fractures in the musculoskeletal system.
  • Sixth, understand the purpose of the study and agree to participate in the study.

排除标准

  • First, those who are unable to stand due to limited ankle range of motion.
  • Second, those with metal implants from calf and ankle surgery.
  • Third, have a pacemaker or a history of heart disease.
  • Fourth, other medical conditions (venous and arterial thrombosis, thrombophlebitis).
