Islatravir (MK-8591) is a next-generation nucleoside analog reverse transcriptase inhibitor (NRTI) that blocks HIV-1 replication by inhibiting reverse transcriptase translocation (immediate chain termination) and by inducing structural changes in viral DNA (delayed chain termination). It is the islatravir component of IDVYNSO, a once-daily single-tablet two-drug regimen with doravirine approved by the FDA in April 2026 for HIV-1 infection in virologically suppressed adults, and serves as the anchor medicine in investigational once-weekly two-drug regimens for HIV-1, including in combination with lenacapavir (Phase 3).
