Novel Framework Predicts Immune System Dysfunction
In a parallel breakthrough, researchers developed the Human Immune Dysregulation Evaluation Framework (HI-DEF), analyzing more than 7,000 blood samples from 37 cohorts across 13 countries. The team, led by Andrew Moore, MD, and senior authors including Khatri, Angela Rogers, MD, and Timothy Sweeney, MD, PhD, collected data from public repositories and a consortium of 11 institutions to understand how immune cell activity predicts critical illness severity and treatment response.
The HI-DEF system provides two scores indicating immune system health status, categorizing patients into four distinct groups: myeloid dysregulation, lymphoid dysregulation, systemwide dysregulation (both myeloid and lymphoid dysfunction), and balanced response (normal immune function). The study demonstrated that increased immune dysregulation scores, characterized by higher levels of "bad" gene signatures compared to "good" ones, correlated with poor outcomes across multiple critical illnesses including sepsis (搜索), burns, trauma, and acute respiratory distress (搜索).
Targeted Treatment Based on Immune Profiles
The research revealed that patients with different types of immune dysregulation respond differently to treatments. "You could have a platform to identify the infection, severity of the illness and the treatment quickly," Khatri stated, describing the potential clinical impact.
Future Applications Beyond Critical Care
The researchers envision expanding the dysregulation scoring system beyond critical illnesses. Since signs of immune dysregulation can appear before patients require ICU care, the tool could serve as a surveillance mechanism for general health concerns. Previous work by Khatri found that patients with high-risk conditions such as diabetes (搜索) exhibited higher numbers of "bad" gene signatures, suggesting broader applications for preventive medicine.
