AL-S Pharma's AP-101 Shows Promising Survival Benefits in Phase 2 ALS Trial, Advancing to Phase 3

AL-S Pharma (搜索) announced positive results from its Phase 2 clinical trial of AP-101, an investigational human antibody therapeutic targeting misfolded superoxide dismutase 1 (SOD1 (搜索)) for the treatment of amyotrophic lateral sclerosis (搜索) (ALS (搜索)). The results were presented at the 36th International Symposium on ALS/MND in San Diego, showing the therapy prolonged survival and delayed the need for ventilatory support in ALS patients.
Phase 2 Trial Results Demonstrate Clinical Benefit
The global, randomized, double-blind, placebo-controlled study enrolled 73 participants and successfully met its primary endpoint related to safety and tolerability. Adverse events were comparable to placebo, and no AP-101-induced antibody responses were reported during the trial period.
Analysis of an exploratory composite endpoint revealed that early treatment with AP-101 prolonged survival and delayed ventilatory support compared to study participants who received placebo for six months followed by six months of AP-101 treatment. The therapy demonstrated positive treatment effects in both the sporadic ALS (搜索) cohort (p = 0.013) and the SOD1 (搜索) mutation carrier cohort (p = 0.036).

Functional and Biomarker Improvements
The survival benefits were accompanied by disease stabilization as measured by King's staging. Functional decline measured by ALSFRS-R was reduced in study participants with elevated misfolded SOD1 (搜索) at baseline and in SOD1 mutation carriers. Additionally, AP-101 treatment resulted in favorable neurofilament biomarker changes that aligned with the observed clinical benefits.
The study design included 52 participants with sporadic ALS (搜索) and 21 with mutant SOD1 (搜索)-ALS, treated over six months followed by a six-month open-label extension and safety follow-up period. The trial evaluated safety, tolerability, pharmacodynamic markers, and pharmacokinetics of the investigational therapy.
Targeting Misfolded SOD1 as Disease-Modifying Approach
AP-101 is designed to selectively target misfolded SOD1 (搜索) to disrupt the progressive spread of pathology in ALS (搜索). The Phase 2 study results support the hypothesis that targeting misfolded SOD1 represents a disease-modifying approach in ALS treatment. The investigational therapy has received orphan drug designations from the FDA, EMA, and Swissmedic.

Advancing to Phase 3 Development
