A diagnosis of alcohol-related liver disease (搜索) (ALD) appears to serve as a powerful catalyst for behavior change, with many patients significantly reducing or stopping alcohol consumption in the immediate aftermath. However, more than one-third of those who achieve initial abstinence relapse within three months, according to a new observational study conducted at Zealand University Hospital in Denmark.
The findings, published in JHEP Reports, reveal both the promise and limitations of leveraging the diagnostic moment as a therapeutic opportunity.
Study Design and Patient Population
Between 2021 and 2025, investigators screened 299 consecutive patients newly diagnosed with ALD, ultimately enrolling 104 who met the study criteria. The cohort had a median age of 64 years, with 76% being men, and 30% had decompensated cirrhosis.
Participants underwent structured interviews covering current and lifetime alcohol consumption, motivation to reduce drinking, and confidence in maintaining abstinence. Researchers also measured phosphatidylethanol (搜索) (PEth), a blood biomarker used to objectively assess alcohol consumption, and followed patients for 90 days.
Sharp Decline in Self-Reported Drinking
In the five years preceding diagnosis, 76% of participants reported consuming at least 20 alcoholic drinks per week, following nearly three decades of escalating alcohol use. Shortly after receiving an ALD diagnosis, that proportion fell to 24%, while 56% reported complete abstinence.
However, PEth biomarker testing told a more sobering story: only 37% of patients were confirmed as abstinent based on blood analysis, indicating that self-reported data alone may overestimate the true rate of cessation.
Relapse Patterns and Predictors of Sustained Abstinence
Among patients who were abstinent at baseline, 37% resumed alcohol consumption within the 90-day follow-up period. Researchers identified several factors associated with sustained abstinence: greater motivation to change, stronger confidence in the ability to avoid alcohol, and lower baseline PEth concentrations.
Notably, neither the severity of liver disease nor Alcohol Use Disorders Identification Test (AUDIT) scores significantly predicted abstinence status at three months. This suggests that psychological and behavioral factors—rather than clinical disease markers—may be more influential in determining early outcomes.
Clinical Implications
The researchers emphasized that the period immediately following an ALD diagnosis represents a critical window for intervention, as many patients demonstrate willingness to change their drinking habits. At the same time, the 37% relapse rate within three months signals that motivation alone may be insufficient to sustain long-term abstinence.
The study authors recommend integrating structured treatment for alcohol use disorder directly into liver care services, rather than relying on brief advice or patient initiative alone.

The researchers acknowledged several important limitations, including the observational study design, partial reliance on self-reported alcohol consumption alongside biomarker testing, and the relatively small sample size drawn from a single center.
