The Mechanism Behind the Allergy
Unlike other tick-borne illnesses such as Rocky Mountain spotted fever, alpha-gal syndrome (搜索) is not caused by a bacterium or virus. Instead, it occurs when the human immune system triggers an allergic response to alpha-gal, a sugar molecule found in the meat of most mammals but absent in humans and other primates. The sugar is also present in the saliva of certain ticks.
When a tick bites through the skin, it can introduce alpha-gal directly into the bloodstream, triggering the development of antibodies that learn to identify and attack the sugar molecules. "It turns out that the skin is a fantastic way to make an allergic response," said Dr. Scott Commins, an alpha-gal syndrome (搜索) researcher at the University of North Carolina. "If this all happened orally, and we were eating alpha-gal like we do with steaks or barbecue, then we wouldn't become allergic."
People who develop these antibodies often experience a strong allergic reaction several hours after consuming meat or dairy. Symptoms can include hives, dizziness, difficulty breathing, swelling of the lips, throat, tongue or eyelids, as well as digestive issues such as diarrhea, stomach pain, vomiting and nausea. The severity of symptoms often increases over time, and it can take weeks or months for the condition to fully develop.
Rising Cases and Expanding Tick Range
Roughly 450,000 Americans are estimated to have developed the allergy, according to a 2023 study by the Centers for Disease Control and Prevention. Experts attribute the growing numbers to both heightened awareness and the expanding geographic reach of the lone star tick.
"I think part of it is more people have learned about it and are on the watch for this syndrome," said Maria Diuk-Wasser, a Columbia University researcher who studies tick-borne diseases.
Often identifiable by a white dot on its back, the lone star tick is most common in the eastern and southern United States. In recent years, however, it has been reported in new parts of the country, including the Great Lakes region and as far north as Martha's Vineyard in Massachusetts. Researchers also worry that other tick species, including blacklegged ticks, may increasingly spread the condition.
Diagnosis and Dietary Management
Diagnosis relies on a combination of blood testing for alpha-gal antibodies, patient-reported symptoms, and details such as whether the individual recalls any recent bug bites. However, Commins cautioned that the blood test alone is insufficient. "The blood test in and of itself is great, but you can't rely on that just for diagnosis. You need the actual symptoms too," he said. "In the allergy world, we have a lot of trouble with false positives on blood tests."
Patients are generally advised to avoid beef, pork, lamb and other mammalian meats. Some individuals can still tolerate dairy products such as milk, cheese and butter, while those with severe reactions may need to avoid foods containing animal byproducts like gelatin. Seafood and poultry, including chicken, turkey and eggs, remain safe to consume.
A rare exception exists: meat from genetically modified "GalSafe" pigs, which have been engineered to not produce alpha-gal. Approved for consumption by the FDA in 2020, these pigs were originally bred as part of an experimental effort to harvest animal organs for transplantation into humans. Meat from these pigs is available through a company called Amaroo Hills (搜索).
Patients may also need to avoid certain medical products and implants, as many heart valves are manufactured from cow or pig tissue.
Xolair and the Treatment Landscape
Xolair, first approved more than 20 years ago for patients with hard-to-control asthma, works by reducing the release of biological chemicals that cause inflammation and other allergic reactions. Its 2024 approval for food allergies, including alpha-gal syndrome (搜索), provides patients with a pharmacologic option to mitigate the risk of severe reactions after accidental exposure.
The allergy can fade away in some individuals after several years. Commins has observed this in approximately 15% to 20% of his patients, though avoiding new tick bites remains critical.
Looking ahead, researchers are exploring whether other previously approved biologic drugs could offer additional treatment avenues. "There are certain biologic drugs out there nowadays that interfere with the allergic signaling," Commins said. "We think that if you were on one of those — or if you got one quickly enough after a tick bite — perhaps it could interfere with the entire allergic response process."
