Aprogen Advances AP209, a Bispecific Receptor Osteoarthritis Drug, into Phase 1 Clinical Trials Following South Korean IND Approval
核心洞察
Aprogen (搜索) has received IND approval from South Korea's Ministry of Food and Drug Safety and submitted its first IRB filing to Kyung Hee University Hospital (搜索) for the osteoarthritis (搜索) candidate AP209 (搜索).
AP209 (搜索) is a bispecific receptor platform-based therapy designed to simultaneously suppress inflammation, inhibit cartilage-destroying osteoclasts, and activate cartilage-forming osteoblasts.
The Phase 1 trial will enroll 46 patients with knee osteoarthritis (搜索), beginning with a single-dose cohort of 16 patients followed by a multi-dose cohort of 30 patients, with first dosing expected in early August.
Aprogen (搜索) has cleared a critical regulatory milestone in the development of AP209 (搜索), a novel bispecific receptor-based therapy for degenerative osteoarthritis (搜索), receiving Investigational New Drug (IND) approval from South Korea's Ministry of Food and Drug Safety. The company has now submitted its first Institutional Review Board (IRB) filing to Kyung Hee University Hospital (搜索), initiating the administrative procedures required to begin patient recruitment for Phase 1 clinical trials.
The IRB submission includes the clinical trial protocol, patient consent forms, a list of trial sites and investigators, case report forms (CRFs), and the investigator's brochure. IRB approval is a mandatory step designed to protect the safety and rights of clinical trial participants; once granted, active patient enrollment can commence at the hospital.
Multi-Center Trial Design and Dosing Strategy
Starting with Kyung Hee University Hospital (搜索), Aprogen (搜索) is in discussions with three additional university hospitals to participate in the clinical trial, employing a multi-center strategy to accelerate patient recruitment and shorten the overall development timeline.
The Phase 1 study will enroll a total of 46 patients with knee osteoarthritis (搜索). The trial is designed in two stages: first, 16 patients will receive a single intra-articular injection of AP209 (搜索) directly into the knee joint, with doses sequentially escalated to confirm safety. Subsequently, 30 additional patients will receive repeated administrations over 11 weeks, followed by observation through 13 weeks to evaluate pharmacokinetics (PK) and initial therapeutic efficacy. The company aims to administer the first patient dose in early August.
Differentiated Mechanism of Action
AP209 (搜索) is built on a bispecific receptor platform, distinguishing it from conventional bispecific antibody approaches. The drug candidate is designed using two types of human-derived receptors that bind to growth factors, characterized by binding properties that more closely mimic the physiological environment. According to the company, comparative studies against bispecific antibodies demonstrated significantly superior therapeutic effects with the bispecific receptor approach.
An Aprogen (搜索) representative explained that AP209 (搜索) "has a mechanism that suppresses the excessive proliferation of sensory nerve cells and inflammatory immune cells that cause osteoarthritis (搜索) pain, while simultaneously inhibiting osteoclasts that destroy cartilage and activating osteoblasts that induce cartilage formation." The representative added, "We expect it to become an innovative treatment that not only alleviates pain but also prevents and restores joint damage itself."
Preclinical Evidence
In non-clinical studies, beagle dogs with induced arthritis showed recovery of gait function to normal levels and improvement in joint damage. Notably, the drug's efficacy was more pronounced in animals with larger joint sizes. Aprogen (搜索) anticipates that in humans, whose knee joint surface area is approximately 13 times larger than that of dogs, therapeutic effects may be reproducible with relatively lower doses and longer dosing intervals.
The company also successfully completed preclinical trials involving more than 150 monkeys, conducted through SNBL (搜索), a prominent preclinical trial agency in Japan, as part of the IND application package.
Market Context and Unmet Need
The degenerative osteoarthritis (搜索) treatment market is projected to experience sustained growth as the global population ages. Current therapies are dominated by analgesics and anti-inflammatory drugs focused on pain relief, along with intra-articular hyaluronic acid injections. However, fundamental treatments that address cartilage regeneration or inhibit disease progression itself remain an unmet medical need. Given that AP209 (搜索) simultaneously targets both inflammation suppression and cartilage regeneration, it has the potential to attract significant market attention depending on clinical outcomes.
With the IRB submission as a starting point, Aprogen (搜索) aims to begin patient dosing within the year and plans to refine its subsequent clinical development strategy based on the results of this Phase 1 trial.
