The four-year PaedNEO-VAX study will enroll approximately 70 children with "dismal prognoses" across eight pediatric hospitals throughout Australia.
The trial targets children who have completely exhausted other therapeutic options, focusing on those with newly diagnosed diffuse midline glioma (搜索) and relapsed, unresponsive high-grade brain tumors including medulloblastoma (搜索) and ependymoma (搜索). Brain cancer (搜索) kills approximately 40 Australian children annually, with survival rates for the worst childhood brain cancers remaining unchanged for decades.
Personalized Treatment Approach
Scientific lead Brandon Wainwright from the University of Queensland (搜索)'s Frazer Institute explained that small samples of each child's tumor will be genetically sequenced and assessed for individual cancer markers (搜索) that could serve as targets for therapy. "After identifying unique cancer markers in each child's tumor using genome sequencing and advanced data analysis, we will select tumor-specific vaccine targets," Wainwright said.
Customized vaccines for each child will be manufactured by Queensland-based company Southern RNA (搜索) within approximately eight to ten weeks of trial enrollment. The treatment protocol involves eight doses injected into the arm, administered two weeks apart, followed by a booster shot several months later.
mRNA Technology Application
The trial harnesses messenger RNA technology proven effective in COVID-19 vaccines, working with Canadian mRNA medicines platform company Providence Therapeutics (搜索). Wainwright, who has spent 30 years researching childhood brain cancers, noted that personalized mRNA cancer vaccines had already shown promising results in adults with pancreatic cancer (搜索) and melanoma (搜索).
"By making the immune system recognise those new and unique proteins that the cancer's making, this allows an opportunity for the immune system to now target and kill those cancer cells," explained Griffith University Professor Nigel McMillan, who described the study design as a "really appropriate approach to take to what is a really devastating disease."
Trial Design and Expectations
Clinical lead Jordan Hansford from the South Australian Health and Medical Research Institute emphasized that all children in the trial will receive a personalized vaccine, with no placebo group. "We don't use placebos. We think that's unethical in children who have very, very poor outcome disease," Hansford said.
The trial will begin with Phase I to assess safety and optimal dosing in the first seven to ten patients before progressing to Phase II to evaluate effectiveness and impact on survival and quality of life. Researchers expect to determine whether there has been a response in individual patients within 12 months of starting treatment.
Hansford expressed cautious optimism based on adult studies: "We're hopeful from the adult studies that at least half of our kids who will be eligible for the study may have some sort of immune response."
Targeting Quality of Life
Beyond survival improvements, researchers emphasize the importance of quality of life for these young patients. "These patients have a variety of terrible symptoms. If we can reduce the severity of disease and improve their overall survival, we're on a path that hasn't been possible at all," Wainwright said.
The trial addresses a critical challenge in childhood brain cancer (搜索) treatment: these cancers are adept at hiding from the immune system and need to be exposed before they can be targeted. The mRNA vaccines are designed to provide the immune system with what McMillan described as an "identikit photo or mug shot" of cancer markers (搜索) specific to each individual tumor.
Funding and Support
The trial has received 2.58 million Australian dollars in federal funding from the Medical Research Future Fund. Additional support comes from the Marcus Rosin Fund, established by Melbourne mother Marisa Rosin after losing her nine-year-old son Marcus to brain cancer (搜索) in 2014.
"If this works, it will be incredible," Rosin said. "If they can just give these kids a little bit more time — whether it be one year, two years, three years with their parents, we've done our job."
The Australian and New Zealand Children's Haematology/Oncology Group will oversee the trial, with hospitals in New South Wales, Victoria, Queensland, South Australia, and Western Australia participating in patient recruitment.
