The OMAN randomized clinical trial, led by Xiaoping Chen, MD, at West China Hospital, Sichuan University, enrolled 720 adults with hypertension across 15 hospitals between June 2022 and April 2024. The study directly compared morning dosing (6:00-10:00 AM) with bedtime dosing (6:00-10:00 PM) using a single-pill combination of olmesartan 20 mg and amlodipine 5 mg.
Superior Nighttime Blood Pressure Control
After 12 weeks of treatment, bedtime dosing demonstrated clear advantages in nocturnal blood pressure management. The bedtime group achieved a significantly greater reduction in nighttime systolic blood pressure, with a between-group difference of -3.0 mm Hg (95% CI, -5.1 to -1.0; P=0.004) compared to morning dosing. Nighttime diastolic blood pressure also showed superior reduction with bedtime dosing (-1.4 mm Hg; 95% CI, -2.8 to -0.1; P=0.04).
The clinical impact was substantial: nocturnal systolic blood pressure control reached 79.0% with bedtime dosing versus 69.8% with morning dosing (P=0.01). Office systolic control also favored bedtime dosing at 88.7% compared to 82.2% for morning dosing.
Safety Profile Remains Favorable
Importantly, the improved nighttime control came without safety trade-offs. Rates of nighttime hypotension and overall side effects were similar between groups, addressing a key concern about bedtime dosing potentially causing excessive nighttime blood pressure drops.

The timing advantage aligns with the pharmacokinetic properties of the medications used. Amlodipine reaches peak levels 6 to 12 hours after administration and has a long half-life, making evening dosing well-suited to influence overnight blood pressure patterns. The study used ambulatory blood pressure monitoring (ABPM) to capture readings every 20 to 30 minutes throughout day and night, allowing researchers to detect stubborn nocturnal hypertension that standard clinic checks might miss. This comprehensive monitoring approach enabled dose adjustments at 4 and 8 weeks based on both ambulatory and office readings.
Reconciling Previous Research
These results complement but don't contradict previous large-scale studies. The UK TIME trial, which followed more than 20,000 participants for over five years, found no advantage for evening dosing regarding major cardiovascular outcomes or mortality. However, that study examined different questions in different populations.

The Chinese trial targeted specific questions about nighttime control using a fixed single-pill combination in patients starting or restarting therapy, while TIME examined long-term events in patients already on established treatment regimens.
Clinical Practice Considerations
For clinicians, these findings support considering bedtime administration as a practical strategy for patients with suboptimal nocturnal blood pressure control. The approach aligns with existing guidelines that encourage simple, adherent treatment plans using single-pill combinations to reduce pill burden.
However, the researchers noted that timing changes should not be made independently by patients, particularly those with conditions like diabetes with autonomic neuropathy, advanced kidney disease, or a history of fainting.
The study's 12-week duration focused on blood pressure behavior rather than cardiovascular events, and longer trials will be needed to determine whether improved nighttime control translates into fewer heart attacks and strokes. Additionally, broader access to validated home and ambulatory monitors would help clinicians identify and treat hidden nocturnal hypertension more effectively.
