The study, published in Eye, utilized the TriNetX Global Collaborative Network to analyze risks for 68 ocular diagnoses across a large patient population. Researchers compared 30,911 patients treated with biologics (搜索) against 35,832 patients who received conventional therapies including methotrexate, cyclosporine, acitretin, apremilast, or dimethyl fumarate.
Consistent Ocular Surface Protection
The analysis demonstrated a consistent association between biologic treatment and reduced risk for ocular surface disease. The protective effect was most pronounced for keratitis (搜索), followed by dry eye disease (搜索) and conjunctivitis (搜索). Blepharitis (搜索) rates were also significantly lower among patients receiving biologic therapies.
"Given the overlap between immune pathways implicated in psoriasis (搜索) and ocular inflammatory disease, it has been hypothesized that biologic therapies may be associated with differences in ocular outcomes," noted Dr. Shoham Kubovsky of Hadassah Medical Center (搜索), Israel, and colleagues. "However, population-level evidence regarding such associations remains limited."
The protective associations remained consistent throughout the entire study duration, spanning from 6 months to 120 months, indicating sustained benefits with long-term biologic use.
Additional Ocular Benefits
Beyond ocular surface diseases, patients treated with biologics (搜索) also experienced lower rates of glaucoma (搜索) and age-related cataracts (搜索). However, researchers noted that other factors may contribute to these findings.
"The attenuation of risk among patients treated with biologics (搜索) may reflect both the anti-inflammatory effects of these agents and a reduced reliance on corticosteroids, although this remains hypothetical," the authors explained.
The relationship proved "more variable" for retinal and vitreous diseases, showing a less clear protective effect in these deeper ocular structures.
Clinical Implications
The findings underscore "the close link between systemic inflammation and ocular surface health," according to coauthors Kubovsky and Dr. Natan Lishinsky-Fischer. They emphasized the importance of considering both the disease itself and treatment choices when evaluating ocular findings in psoriasis (搜索) patients.
"When evaluating patients with psoriasis (搜索), it is important to recognize that both the disease itself and the systemic therapies used to treat it may influence ocular findings," they stated. "In some cases, persistent or unexplained ocular surface disease in a patient with psoriasis may warrant communication with the treating physician regarding systemic disease control, although further research is needed to better understand this relationship."
Future Research Directions
The researchers called for prospective studies with "detailed ophthalmic examinations" to provide more precise evaluation of ocular surface findings in psoriasis (搜索) patients receiving different treatments.
"Another interesting question is whether ocular manifestations could potentially reflect systemic disease activity or response to therapy," Kubovsky and Lishinsky-Fischer noted. "Exploring this possibility would require carefully designed clinical studies."
The study represents the largest population-level analysis to date examining the relationship between psoriasis (搜索) treatments and ocular outcomes, providing valuable real-world evidence for treatment decision-making in clinical practice.
