These findings highlight critical gaps in cancer screening and detection that could worsen as healthcare access barriers continue to grow.
Screening Disparities Compound Detection Challenges
The disparities are particularly pronounced for ovarian cancer (搜索), where no effective screening tools currently exist. "There is no good screening tool" for ovarian cancer, explained CK Wang (搜索), MD, a hematologist-oncologist and chief medical officer at COTA. "Therefore, the screening guidelines are not really in place."
This absence of screening protocols means patients typically present with vague abdominal symptoms including discomfort, bloating, or pain. "Most patients are diagnosed with relatively advanced disease just because of the lack of screening tools," Wang noted.
For breast cancer (搜索), while screening mammography has been established for many years, the guidelines vary depending on age and risk factors. However, the data suggest that existing screening protocols are not adequately addressing racial disparities in early detection.
Workforce Shortages Threaten Care Access
The challenge extends beyond screening to treatment access, with significant shortages in specialized care providers. Current estimates show only 1,300 to 1,500 gynecologic oncologists practicing in the United States, which is "significantly smaller than the demand," according to Wang.
This shortage is particularly concerning given that "the first step to the treatment of almost any cancer is surgery," and specialty-trained surgeons are essential for optimal outcomes. "Without access to that definitive care or standard of care, everything else subsequent to that, in terms of outcomes, will be less than ideal," Wang emphasized.
Post-Roe Impact on Medical Training
The healthcare workforce challenges are being exacerbated by declining ob-gyn residency applications following the overturning of Roe v Wade. Wang described seeing "a humongous drop right after Roe was overturned," with only a slight uptick in 2023 that still represents an overall trending decline.
The geographic variation in application rates is particularly troubling. "The rate of application varies dramatically depending on the region of the country," Wang noted, suggesting that certain areas may face more severe shortages than others.
Rural Areas Face Greatest Risk
Rural areas, where access to specialists is already limited, are expected to bear the greatest impact of these trends. The declining number of specialized providers means that "the onus and burden of care of women's cancers will lie primarily in the detection space in the hands of primary care providers, and in the treatment space, it would be the general oncologists who are practicing in those areas."
While general oncologists are trained to treat various cancers, the complexity of gynecologic cancers often requires specialized surgical expertise that may not be readily available in underserved areas.
Geographic and Systemic Barriers
The COTA analysis also revealed that geographic and systemic differences in access to care compound these disparities. Women in states that restrict abortion access are experiencing the highest mortality rates for breast and cervical cancers, indicating that policy environments may influence cancer outcomes beyond direct reproductive health impacts.
Research and Treatment Advances
Despite these challenges, Wang expressed optimism about recent therapeutic developments. Ovarian cancer (搜索) has historically been "a relatively underfunded cancer when it comes to research," with "very few therapeutic advances in this space" until recently. However, "over the last 2 years, there have been new drugs and new therapies coming to the market."
Wang emphasized the importance of including underrepresented populations in clinical trials to "better understand how their care is being given in the real world and whether or not their care actually conforms to what we consider to be our standards."
The findings underscore the urgent need for systemic interventions to address both immediate screening disparities and longer-term workforce challenges that threaten to widen existing gaps in cancer care access and outcomes.
