The landscape of cancer screening is undergoing a fundamental shift as blood-based biomarker tests move from concept to clinic. Two major developments—multi-cancer early detection (MCED) testing and blood-based colorectal cancer (搜索) screening—are generating both excitement and debate among clinicians, regulators, and patients. Yet the central question remains unanswered: are these tests saving lives, or simply making earlier diagnoses?
The MCED Frontier: Galleri (搜索) and the Pathfinder Studies
The promise of blood-based cancer screening rests on a simple biological premise: tumors shed trace amounts of DNA, proteins, and other molecules into the bloodstream before symptoms appear. Detecting those signals could, in theory, identify people who need closer evaluation even when they feel well.
Grail, a company founded after a 2015 JAMA report identified cell-free DNA (搜索) consistent with underlying cancer in asymptomatic pregnant women, has been the driving force behind MCED testing. Its validated test, Galleri (搜索), can identify a biochemical signature associated with up to 50 cancers.
The Pathfinder I study enrolled more than 6,000 adults aged 50 or older with no cancer symptoms. Galleri (搜索) detected a cancer signal in 1.4% of participants. Among those flagged, cancer was confirmed in 38%, while 62% turned out to be false positives. Reaching a definitive answer often required extensive follow-up: additional lab work in 80% of flagged patients and imaging in 90%. Confirming cancer took about two months, but clearing a false alarm took nearly three times as long. The test's negative predictive value reached 99%, but its positive predictive value (PPV) was approximately 40%.
Pathfinder II, which included 23,000 cancer-symptom-free individuals at elevated risk based on lifestyle and family history, has not yet been peer-reviewed. However, reported findings suggest that enriching the screening population raised Galleri (搜索)'s PPV to 62%—a notable improvement and, according to investigators, stronger than some standard screening benchmarks.
NHS-Galleri (搜索): The Largest Real-World Test
The most consequential data come from the NHS-Galleri (搜索) trial, reported at ASCO. This study evaluated 142,000 cancer-asymptomatic adults aged 50–79, with half receiving annual Galleri testing. In this real-world setting, the PPV was 52%—not as strong as Pathfinder II had suggested.
The trial increased the number of cancers identified fourfold compared with the control group receiving standard care. It found more early-stage cancers (16%) and reduced late-stage cancers, defined as Stage IV with distant spread, by 14%. Despite these shifts in stage distribution, there is no information yet on whether the test reduced mortality.
This distinction matters critically. Earlier diagnosis can help, but it can also introduce lead-time bias, overdiagnosis, anxiety, invasive follow-up testing, and cost. For regulators and clinicians, the central question is whether Galleri (搜索) is ready for routine care or requires further study to demonstrate that its benefits outweigh its financial and psychological harms.
Shield (搜索): A Blood Test for Colorectal Cancer (搜索)
Parallel to MCED development, Guardant Health's Shield (搜索) has emerged as the first and currently only FDA-approved blood-based colorectal cancer (搜索) screening test for average-risk adults aged 45 and older. In a clinical trial of more than 20,000 participants, Shield demonstrated 83% sensitivity for colorectal cancer and 90% specificity.
Dr. Craig Eagle, former Chief Medical Officer at Guardant Health, explains the mechanism: when a colorectal tumor is present, it continuously sheds tiny fragments of its DNA into the bloodstream. Shield (搜索) analyzes a blood sample for those fragments, looking for cancer-specific patterns. The test can be ordered by a primary care doctor as an outpatient, and the company recommends testing every three years. It is currently covered by Medicare.
Eagle is careful to set expectations: "Colonoscopy remains the gold standard." A positive Shield (搜索) result still requires a follow-up colonoscopy. Where Shield stands out is as a lower-friction option for the millions who have not completed a colonoscopy on schedule due to fear, access, scheduling, or cost. "We have the best test available to all, colonoscopy, and only 20 to 40% of people actually complete it, depending on the study," Eagle notes. "No matter how good the test is, if it's not done, it's a waste of time."
The American Cancer Society's updated 2026 guidelines now recommend blood-based testing as a screening option for those who forgo colonoscopy or visual examinations, though they classify it as secondary to colonoscopy and stool tests. The FDA label uses stronger language, designating Shield (搜索) as a primary screening option—a gap that reflects ongoing clinical debate.
Limitations and Trade-offs
Shield (搜索) has important limitations. The test demonstrates limited detection (55%–65%) of Stage I colorectal cancer (搜索) and does not detect 87% of precancerous lesions. One out of 10 patients with a negative Shield result may have a precancer that would have been detected by a screening colonoscopy. Shield is not indicated for patients with a personal history of colorectal cancer, adenomas, inflammatory bowel disease, chronic ulcerative colitis, Crohn's disease, family history of colorectal cancer, or certain hereditary syndromes.
A Case That Illustrates the Stakes
Kim Turner, a 61-year-old physician assistant in Alaska, had her first colonoscopy at 50, which found a one-centimeter precancerous polyp. Her follow-up at 54 was clear. With no family history of cancer, obesity, or smoking, her gastroenterologist told her she did not need another colonoscopy for 10 years.
In 2025, Turner's daughter organized a health fair and suggested a Shield (搜索) test. "I didn't expect it to come back positive," Turner said. A follow-up colonoscopy confirmed adenocarcinoma in a three-centimeter section of her sigmoid colon. She had no rectal bleeding or abdominal pain. The only symptom was very mild, intermittent constipation attributed to an unrelated condition. "I effectively had no symptoms," she said. "I was shocked."
Surgery removed the cancerous mass along with 28 lymph nodes, one of which was positive, classifying her as Stage 3. At the time of her interview, she was undergoing a twelve-week chemotherapy regimen. "If I would have waited three years," she said, "very different story. Very different."
The Screening Gap and the Path Forward
Colorectal cancer (搜索) is the second leading cause of cancer-related death in the United States, yet more than 45 million eligible Americans are due or overdue for screening. While colonoscopy remains the gold standard, only 20% to 40% of patients follow through with the entire procedure. For average-risk adults with a clean colonoscopy, guidelines recommend returning in up to 10 years—an interval that Turner's case calls into question, particularly for patients with a prior polyp history.
The balance between over-screening and missing disease remains a clinical judgment call that guidelines are still calibrating. False positives carry real costs: unnecessary procedures, anxiety, and expense. No blood or stool test matches the sensitivity of a well-performed colonoscopy. Yet in a world with a screening deficit measured in the millions, getting screened by whatever means a patient will actually complete may eventually matter more than which test they choose.
