Flagellin Antibody Response as Predictive Biomarker
The test measures a person's immune response to flagellin, a protein found on gut bacteria. Dr. Ken Croitoru, a clinician scientist at the Lunenfeld-Tanenbaum Research Institute (搜索) and Canada Research Chair in Inflammatory Bowel Diseases, led the research team that discovered this response is elevated in individuals long before they develop Crohn's disease (搜索).
"The presence of flagellin antibodies long before any symptoms appear suggests that this immune reaction may contribute to triggering the onset of the disease, rather than being a consequence of it," Dr. Croitoru explained. This finding challenges conventional understanding of disease progression and opens new avenues for therapeutic intervention.
Clinical Study Results
The research followed 381 first-degree relatives of Crohn's patients as part of the Genetic, Environmental and Microbial (GEM) Project, a global cohort of more than 5,000 healthy first-degree relatives led by Dr. Croitoru since 2008. Among the study participants, 77 individuals eventually developed Crohn's disease (搜索), with 28 of them—more than a third—showing elevated antibody responses to flagellin.
The typical timeline from blood sample collection to Crohn's diagnosis was nearly two and a half years, providing a substantial window for potential intervention. The responses were strongest in siblings, highlighting the role of shared environmental exposure as previously demonstrated by Dr. Croitoru's team.
Disease Mechanism Insights
The research builds on previous work by collaborators at the University of Alabama (搜索) led by Dr. Charles Elson, who developed a test to detect antibodies against flagellin and showed that individuals with Crohn's have elevated antibody levels targeting flagellin from Lachnospiraceae (搜索) bacteria.
"We wanted to know: do people who are at risk, who are healthy now, have these antibodies against flagellin? We looked, we measured, and yes indeed, at least some of them did," Dr. Croitoru said.
The team confirmed that this pre-disease response to Lachnospiraceae (搜索) flagellin was associated with intestinal inflammation and gut barrier dysfunction, both characteristic features of Crohn's disease (搜索). In healthy individuals, bacteria coexist peacefully in the gut and play an essential role in maintaining digestive health. However, in Crohn's disease, the immune system mounts an abnormal response against normally beneficial microbes.
Clinical Implications and Future Directions
Dr. Sun-Ho Lee, clinician scientist and staff gastroenterologist who co-led the research, emphasized the potential for prevention strategies. "We found that this immune response is driven by a conserved domain of the flagellin protein. This raises the potential for designing a flagellin-directed vaccine in selected high-risk individuals for prevention of disease."
The research addresses a critical unmet medical need, as Crohn's disease (搜索) incidence among children has doubled since 1995, with rates continuing to rise. Crohn's and Colitis Canada (搜索) estimates about 470,000 Canadians will be living with inflammatory bowel disease (搜索) by 2035.
"With all of the advanced biologic therapy we have today, patients' responses are partial at best. We haven't cured anybody yet, and we need better," Dr. Croitoru noted, highlighting the limitations of current treatment approaches.
Research Foundation
The GEM Project has collected genetic, biological and environmental data since 2008 to better understand disease development. To date, 130 participants have developed Crohn's, providing researchers with a rare opportunity to study the earliest pre-disease stages. The team previously discovered that long before Crohn's disease (搜索) develops, an inflammatory immune response targeting gut bacteria can appear.
The research team, which included gastrointestinal medical resident Dr. Richard Wu, is part of Mount Sinai Hospital's Centre for Inflammatory Bowel Disease (搜索), a globally recognized center focused on inflammatory bowel disease research. Further validation and mechanistic studies are currently underway to advance this promising diagnostic and preventive approach.
