Brain Injury Survivors Face 1.75-Fold Higher Brain Cancer Mortality Risk, Landmark 37-Year Study Finds

A sweeping analysis of nearly four decades of patient data has revealed that individuals who sustain a traumatic brain injury (搜索) (TBI) face a significantly elevated risk of dying from brain cancer (搜索), challenging the traditional view of TBI as an isolated acute event and underscoring the need for lifelong monitoring of survivors.
The study, published in Neuroepidemiology and led by researchers at Mass General Brigham (搜索)'s HealthSpan Lab, found that people with a history of TBI were 1.75 times more likely to die from brain cancer (搜索) than the demographically matched general population (SMR=1.75; 95% CI: 1.04–2.77).
"This study examined whether individuals who sustain a TBI are more likely to die from brain cancer (搜索) compared to the general population, and whether certain factors modify that risk," said Daniel Daneshvar, MD, PhD, director of the HealthSpan Lab and Chief of the Department of Physical Medicine and Rehabilitation at Mass General Brigham (搜索), who served as senior author. "By focusing on mortality rather than incidence alone, our study addressed a key gap: even if TBI increases the likelihood of developing a brain tumor, it has been unclear whether it translates into worse survival outcomes at the population level."
Gunshot Wounds and Mild Complicated TBI Carry Outsized Risk
The most alarming finding emerged from subgroup analyses by injury mechanism. Individuals whose TBI resulted from gunshot wounds were over 14 times more likely to die from brain cancer (搜索) compared to matched controls (SMR=14.29; 95% CI: 2.95–41.75).
Even patients with so-called "mild but complicated" TBI—head injuries that display intracranial abnormalities such as localized bleeding on neuroimaging—exhibited a nearly four-fold increase in brain cancer (搜索) mortality risk (SMR=3.88; 95% CI: 1.06–9.94). Moderate-to-severe TBI showed elevated but statistically nonsignificant risk (SMR=1.52; 95% CI: 0.83–2.54), which the authors attribute to smaller sample sizes in those subgroups.
A 37-Year National Database Effort
The investigation drew on the TBI Model Systems National Database, a longitudinal cohort of more than 20,000 civilians who received inpatient rehabilitation across 20 specialized centers—including Spaulding Rehabilitation Hospital—between 1987 and 2024. Brain cancer (搜索) deaths were identified using underlying or contributing cause of death on death certificates.
The research team, which included lead author Charlotte Luster of the HealthSpan Lab alongside co-authors Shanti Pinto, Mitra McLarney, Evan D. Feigel, Saef Izzy, Brian Nahed, and Joseph Giacino, compared observed deaths in the TBI cohort to expected deaths in a demographically matched general population, adjusting for age, sex, race, and calendar year. Secondary analyses explored cancer risk factors such as smoking and alcohol use.
Of the 4,297 decedents in the cohort, 18 died of brain cancer (搜索)—a rate that proved significantly higher than would be expected in the general population.
The Inflammatory Malignancy Hypothesis
While the study does not establish causation, the findings add weight to a growing body of evidence suggesting that the chronic inflammation triggered by TBI may have lasting biological effects. Researchers hypothesize that the initial mechanical impact unleashes persistent neural inflammation and metabolic dysregulation, creating a biological microenvironment that may inadvertently fuel oncogenesis.
"These findings suggest that certain groups of TBI survivors may benefit from closer long-term monitoring for brain cancer (搜索)," Daneshvar said. "For clinicians, the results reinforce that TBI should not be viewed solely as an acute event, but as a condition with potential long-term neurological consequences, including malignancy."
Implications for Clinical Practice and Future Research
The study stops short of recommending immediate changes to clinical guidelines, but the authors argue it strongly supports further research into cancer surveillance among TBI survivors and underscores the value of linking injury registries with cancer databases to better track long-term outcomes.
"For patients and families, the study highlights the importance of ongoing follow-up care after serious brain injury," Daneshvar noted.
The HealthSpan Lab is now expanding its predictive model to investigate other long-term outcomes following TBI, including neurodegenerative conditions such as amyotrophic lateral sclerosis (搜索) (ALS). Future work will explore how biological, behavioral, and environmental factors interact to shape long-term risk, with a particular focus on identifying markers of resilience and vulnerability as well as opportunities for early intervention to improve healthspan—the period of life spent in good health.
