Brazil Approves World's First Single-Dose Dengue Vaccine Following Severe 2024 Outbreak

Breakthrough Efficacy Results
In late-stage human trials involving 16,000 volunteers from across Brazil, the vaccine demonstrated 74.7% overall efficacy and 91.6% efficacy against more severe forms of dengue (搜索). The most recent data indicates the vaccine achieved 100% efficacy against hospitalizations, with results set to be published in Nature Medicine.
The vaccine was evaluated for nearly a decade in trials, with previous results published in The New England Journal of Medicine and The Lancet Infectious Diseases after two and 3.7 years of follow-up.
Addressing Brazil's Dengue Crisis
The approval comes after Brazil experienced its largest ever dengue (搜索) epidemic in 2024, with 6.4 million cases and 5,972 deaths reported by the Ministry of Health. Esper Kallas, director of the Butantan Institute, called the vaccine a "powerful weapon" against the disease that has plagued Brazil for decades.
The vaccine will be incorporated into Brazil's national immunization program in early 2026, with more than 1 million doses ready for distribution. Butantan has partnered with Chinese company WuXi Vaccines (搜索) to manufacture 60 million doses over the next two years, with around half expected to be delivered before the end of 2026.
Transforming Access in Remote Regions
Disease specialists emphasize that the single-dose format will revolutionize vaccine access in Brazil's remote communities, particularly in the Amazon region. "It is much more difficult and expensive to apply a two-dose vaccine in remote and hard-to-reach regions, such as the Amazon, crossed by rivers and with still very precarious land connections," explained epidemiologist Jesem Orellana from Brazil's Oswaldo Cruz Foundation.
Indigenous communities will no longer need to make repeated trips to health clinics and can be fully vaccinated in one community health visit, a significant improvement over the only globally available vaccine, TAK-003 or Qdenga, which requires two doses administered three months apart.
Vaccine Technology and Advantages
Butantan-DV (搜索) is a live, attenuated vaccine that uses weakened viruses to generate immunity without causing disease. The vaccine is composed of all four serotypes of the dengue virus (搜索) and can be administered to both people who have previously had dengue (搜索) and those who have never been exposed to it.
This sets it apart from other dengue (搜索) vaccines approved by Brazil's health regulatory agency Anvisa. According to physician and virologist Maurício Nogueira from the Faculty of Medicine of São José do Rio Preto, the first vaccine, Dengvaxia from Sanofi Pasteur, "could only be administered to people who had already had dengue, which prevented its integration into the public health system."
Takeda (搜索)'s Qdenga vaccine is suitable for people who have had dengue (搜索), but its two-dose format creates adherence challenges. "Many people who receive the first dose don't return for the second and end up only partially protected," Nogueira explained. "Both doses are essential to ensure complete protection against the pathogen."
Global Impact and Future Distribution
The vaccine approval positions Brazil as a strategic player in international vaccine production. "With this, Brazil becomes a strategic player in the international production of vaccines, and will soon be able to supply other Latin American countries also affected by the disease, such as Argentina, Peru and Colombia," said Renato Kfouri, vice president of the Brazilian Society of Immunizations.
The Butantan Institute confirmed it will be able to offer the vaccine to other countries in the region, though no specific timescale was provided. Fernanda Boulos, Butantan's chief scientific officer, said the priority remains supplying Brazil's public health system through the Ministry of Health.
Research Partnership and Development
The vaccine's development involved significant contributions from international partners, including 16 years of research by scientists at the University of Vermont Vaccine Testing Center, in partnership with the U.S. National Institutes of Health and Johns Hopkins Bloomberg School of Public Health. More than 27 clinical trials have been conducted since 2009 to develop this vaccine.
Current Limitations and Ongoing Research
The vaccine is currently approved for people aged 12 to 59 and is not yet authorized for pregnant women, immunocompromised people, or older adults. Evidence of its efficacy in these populations is being analyzed by Brazil's health regulatory agency.
Continued Need for Vector Control
Disease specialists warn that vaccination alone will not eradicate dengue (搜索) and stress the continued importance of mosquito control measures. Climate change and urbanization have created increasingly favorable conditions for the Aedes aegypti (搜索) mosquito, the primary disease vector.
"The vaccine is important, but it doesn't mean we should neglect mosquito control," said Orellana, citing the risk of other mosquito-borne diseases such as chikungunya and yellow fever. "Control remains essential, surveillance is fundamental, and it is necessary to invest in measures that reduce the vector's reproductive capacity."
Globally, more than 14 million dengue (搜索) cases were reported in 2024 according to the World Health Organization, with 12.6 million occurring in Latin America where more than 8,000 people died from the disease.
