Brazil Launches World's First Pediatric Tafenoquine Treatment for Malaria in Indigenous Communities

The timing of this rollout addresses an urgent public health crisis in the Yanomami Indigenous Territory, where malaria (搜索) cases have surged 300% between 2016 and 2022. This dramatic increase has been driven by illegal mining and deforestation, creating conditions that favor the mosquitoes carrying the disease. The Yanomami, Brazil's largest indigenous population, live in villages within the Amazon rainforest on the border of Venezuela and Brazil.
The severity of the situation prompted Brazil to declare malaria (搜索) a public health emergency in the area in 2023. Between 2019 and 2022, over 21,000 malaria cases were recorded among Yanomami children under the age of five, alongside a sevenfold increase in malaria-related child deaths over four years. In a population of around 30,000 people, these figures underscore the devastating impact of malaria and the urgent need for effective treatment for the youngest and most vulnerable patients.
Treatment Innovation and Advantages
The new pediatric tafenoquine formulation offers several critical advantages over existing treatments. Most significantly, it requires only a single dose, replacing older medicines that must be taken daily for seven days. This is particularly important in remote areas where following through with longer treatment regimens can be challenging.
"The rollout of paediatric tafenoquine addresses a long‑standing gap in malaria (搜索) care for children, particularly in remote and underserved settings. This initiative strengthens our national elimination efforts while centring children's health," said Alex Vargas from Brazil's Ministry of Health.
The single-dose approach provides greater comfort and convenience for families and healthcare professionals, improved adherence to therapy, complete elimination of the parasite, and prevention of relapses, contributing to the interruption of disease transmission. The formulation also allows for dose adjustment according to the child's weight, ensuring greater treatment efficacy.
Distribution Strategy and Early Impact
The Ministry of Health has received 64,800 tablets for distribution in areas with the highest incidence, focusing on Special Indigenous Health Districts (DSEI) including Yanomami, Alto Rio Negro, Rio Tapajós, Manaus, Vale do Javari, and Médio Rio Solimões and Tributaries. These territories account for approximately 50% of malaria (搜索) cases in children and young people up to 15 years old. The Yanomami DSEI will be the first to receive the tablets, with an initial allocation of 14,550 tablets.

Clinical Significance and Safety Considerations
Tafenoquine represents the first new drug developed in over 60 years to treat relapsing malaria (搜索). Without liver-stage treatment, patients infected with P. vivax remain at risk of repeated clinical relapses. However, the drug can cause hemolytic anemia in people with glucose-6-phosphate dehydrogenase (搜索) (G6PD) deficiency, requiring G6PD testing before prescription, which adds complexity to the rollout process.
MMV CEO Dr Martin Fitchet emphasized the treatment's importance, stating that "for too long, younger patients have remained vulnerable to debilitating relapse simply because existing treatments were hard to complete. The rollout of paediatric tafenoquine among the Yanomami marks a major step toward more equitable access to effective malaria (搜索) treatment."
Broader Public Health Impact
Brazil was the first malaria (搜索)-endemic country to adopt tafenoquine for adults into its public health system in 2023, with rollout beginning in June 2024. By April 2025, coverage extended to 49 municipalities from six states and nine Special Indigenous Health Districts.
Early results demonstrate the treatment's effectiveness. In the first quarter of 2025, Brazil recorded a 26.8% drop in all malaria (搜索) cases. Rondônia, one of the first states to introduce tafenoquine treatment, saw a 47% decrease in malaria cases during the first four months of 2025 compared to the same period the previous year, with 95% of all malaria cases during this period being P. vivax.
In the Yanomami territory specifically, between 2023 and 2025, there was a 103.7% increase in testing, a 116.6% increase in the number of diagnoses, and a 70% reduction in deaths from the disease.
According to Mariângela Batista Galvão Simão, Secretary of Health and Environment Surveillance at the Ministry of Health, expanding treatment coverage could significantly reduce disease transmission. "If we can achieve high coverage, it is possible to reduce cases of the disease by up to 20,000," she explained, emphasizing the importance of bringing these technologies to the areas most affected by malaria (搜索) where they can have the greatest impact.
