A comprehensive analysis of 497 fibromyalgia (搜索) patients from the UK Medical Cannabis Registry (搜索) revealed that cannabis-based medicinal products (搜索) (CBMPs) yielded modest, medium-term improvements in multiple patient-reported outcomes. The study, published in Clinical Rheumatology, tracked patients over 18 months using validated assessment tools including the Fibromyalgia Symptom Severity scale and Fibromyalgia Widespread Pain Index.
"All patient-reported outcome measures improved from baseline through all follow-up time points (P < .01)," reported Madhur Varadpande, MBBS, from Imperial College London's Medical Cannabis Research Group. The improvements extended beyond pain to include anxiety, sleep quality, and general quality of life measures assessed at 1, 3, 6, 12, and 18-month intervals.
The registry data identified key factors associated with better outcomes. Cannabinoid doses greater than 25 mg per day and previous cannabis use were both linked to an increased likelihood of improved fibromyalgia (搜索)-specific measures (P < .05). This dosage finding may prove significant given that an estimated 1 in 2 patients with fibromyalgia consume cannabis, with 82% of users reporting pain improvement in cross-sectional studies.
Safety Profile Reveals High Adverse Event Rate
Despite the therapeutic benefits observed, the UK study documented a substantial adverse event profile. Of the 497 patients, 227 individuals (45.67%) reported a total of 2,100 adverse events, representing a rate of 422.54%. However, most events were classified as mild to moderate, occurring in 1,792 patients (85.33%). Fatigue emerged as the most frequently reported adverse event, affecting 153 participants (30.78%).
The researchers attributed the high incidence of adverse events to fibromyalgia (搜索)'s central sensitization mechanism, which may increase patient susceptibility to side effects from cannabis-based treatments.
Danish Trial Challenges CBD Efficacy
In stark contrast to the registry findings, a randomized, double-blind, placebo-controlled trial conducted in Denmark found no significant analgesic benefit from cannabidiol in fibromyalgia (搜索) patients. The study, published in Annals of the Rheumatic Diseases, evaluated 200 patients who received either 50 mg of plant-derived CBD or matching placebo over 24 weeks.
Marianne Uggen Rasmussen from the Parker Institute (搜索) at Bispebjerg and Frederiksberg Hospital (搜索) led the Danish investigation, which employed rigorous methodology including randomization stratified by sex, age, and baseline pain levels. The primary outcome measured changes in pain intensity using the numeric rating subitem of the revised Fibromyalgia (搜索) Impact Questionnaire.
The results favored placebo over CBD treatment. At week 24, the CBD group showed a mean reduction in pain intensity of -0.4 points, while the placebo group experienced a greater reduction of -1.1 points. The between-group difference of -0.7 points favoring placebo was not considered clinically meaningful, but provided no support for CBD's analgesic properties in this population.
Methodological Differences May Explain Contrasting Results
The divergent findings between the two studies likely reflect fundamental differences in study design and cannabis formulations. The UK registry analysis examined real-world use of various cannabis-based medicinal products (搜索) containing multiple cannabinoids, while the Danish trial specifically tested a standardized 50 mg dose of isolated CBD.
The registry study's observational design captured patients' self-selected dosing regimens and product choices, potentially explaining why higher doses above 25 mg daily showed superior outcomes.
