The case, reported today, represents the first successful treatment of three simultaneous autoimmune conditions with a single CAR-T intervention.
The patient presented with autoimmune hemolytic anemia (搜索), immune thrombocytopenia (搜索), and antiphospholipid syndrome (搜索)—a life-threatening combination that had left her bedridden and requiring intensive medical support. "Her disease got completely out of hand" and became "very life-threatening," says Fabian Müller, a hematologist at University Hospital Erlangen who helped treat the patient.
Critical Clinical Presentation
The woman's condition stemmed from rogue B cells (搜索) producing antibodies that simultaneously attacked multiple targets in her body. Her immune system was destroying red blood cells, causing severe anemia that required blood transfusions averaging one bag daily, with peaks of up to three bags per day. Simultaneously, antibodies were attacking her platelets, increasing bleeding risk, while also targeting fat-binding proteins, paradoxically raising her risk of dangerous blood clots.
"That combination can kill you very rapidly," notes Carl June, an immunologist at the University of Pennsylvania who pioneered CAR-T cell therapy for cancer. The patient had previously failed nine different treatment approaches, including steroids (搜索) and advanced immunosuppressive medications, and could no longer work due to weeks of debilitating pain and fatigue.
Breakthrough Treatment Approach
Facing treatment failure with conventional therapies, the patient was transported by ambulance for three hours to Müller's clinic, which has developed expertise in manufacturing CAR-T cells (搜索) for autoimmune conditions. "It was her last chance for controlling the disease," Müller explains.
The treatment involved harvesting the patient's own T cells and genetically engineering them to target CD19 (搜索), a protein found exclusively on B cells (搜索). These modified CAR-T cells (搜索) were designed to eliminate the antibody-producing B cells responsible for her autoimmune attacks. The patient also received two chemotherapy drugs (搜索) that target fast-dividing immune cells.
Remarkable Clinical Response
The results exceeded expectations. "She was deathly sick and bedridden at the time we met her, and we treated her, and seven days later, she got out of bed," Müller reports. Within one month of treatment, the patient had normal levels of red blood cells, and within months appeared fully recovered.
Eleven months post-treatment, the patient remains symptom-free and medication-free. "I just saw her yesterday. She's perfectly fine," says Müller. The patient's immune system has effectively reset itself—the CAR-T cells (搜索) were eventually eliminated by her immune system as foreign, allowing new, healthy antibody-producing cells to form without the problematic autoimmune components.
Preserved Immune Function
Importantly, the treatment didn't completely erase the patient's immune system. Her long-lived antibody-producing cells against childhood infections and vaccinations, which reside in bone marrow, remained unaffected. "They sit in the bone, they're unaffected," Müller explains, ensuring continued protection against previously encountered pathogens.
Broader Implications for Autoimmune Treatment
This case adds to growing evidence supporting CAR-T therapy for autoimmune conditions, with promising results reported for lupus (搜索), multiple sclerosis (搜索), colitis (搜索), and severe asthma (搜索). Unlike cancer applications, CAR-T therapy for autoimmune diseases appears to cause fewer serious side effects, possibly because fewer cells need to be eliminated.
"For a therapy that's very powerful to have very few side effects in this particular case, and then to result in resolution of the underlying condition, is pretty remarkable," says Reuben Benjamin at King's College London. "This is a fantastic result."
Economic and Clinical Considerations
While CAR-T treatments carry high upfront costs—ranging from $200,000 to $600,000 for cancer applications—Müller argues the economics favor this approach for severe autoimmune diseases. "It costs a lot to start with, but you save a lot of money in the long run," he notes, citing both reduced ongoing treatment costs and patients' ability to return to productive work.
However, researchers emphasize the need for longer follow-up data. "Longer follow-up is needed before anyone can speak confidently about cure," says Jun Shi at the Chinese Academy of Medical Sciences in Tianjin, whose team is conducting an ongoing trial of CAR-T therapy in 15 people with autoimmune hemolytic anemia (搜索).
The case demonstrates the potential for CAR-T therapy to address multiple autoimmune conditions simultaneously, offering hope for patients with complex, treatment-refractory autoimmune diseases who have exhausted conventional therapeutic options.
