The patient, who had lived with the conditions for more than a decade and required daily blood transfusions, has been in treatment-free remission for over 14 months following treatment at University Hospital Erlangen in Germany.
Unprecedented Multi-Disease Response
The woman suffered from three distinct autoimmune conditions with conflicting effects on her blood system. She had severe autoimmune hemolytic anemia (搜索) (AIHA), where the immune system destroys red blood cells, requiring regular blood transfusions. Additionally, she was diagnosed with immune thrombocytopenia (搜索) (ITP), which caused her immune system to destroy platelets and increase bleeding risk, and antiphospholipid antibody syndrome (搜索), which raised the risk of dangerous blood clots.
"The patient had no treatment options left and would not have left the ward as she needed daily transfusions," said Prof Fabian Müller, who led the treatment team at University Hospital Erlangen. The patient had tried nine different treatments over the years, none of which provided lasting relief.
CAR-T Therapy Mechanism and Implementation
The medical team employed CAR (chimeric antigen receptor) T-cell therapy, a "living drug" approach previously used primarily for blood cancers. The doctors extracted the woman's white blood cells and isolated her T-cells (搜索), which naturally patrol the body to identify and destroy infected or abnormal cells. These T-cells were then genetically engineered to recognize CD19 (搜索), a protein found on B-cells (搜索), before being re-infused into the patient.
The therapy targeted the root cause of all three conditions: wayward B-cells (搜索) that produce harmful antibodies. "The treatment was extremely efficient in getting rid of all three autoimmune conditions at once," Müller explained.
Rapid Clinical Improvement
The response was swift and comprehensive. Within one week of treatment, the patient received her final blood transfusion. Two weeks later, she was strong enough for everyday activities, and by three weeks, her hemoglobin levels had doubled and returned to normal, indicating her immune system had stopped attacking red blood cells.
Simultaneously, her other autoimmune conditions improved. Antiphospholipid antibody levels, associated with blood clot formation, gradually decreased and remained negative. Her platelet counts also stabilized, addressing the bleeding risk from ITP.
"The speed and depth of the response was remarkable," Dr Müller noted. When the patient's B-cells (搜索) regenerated months later, they appeared healthy, suggesting the therapy had successfully reset her immune system.
Current Status and Future Implications
The patient has maintained treatment-free remission for 14 months, with significantly improved quality of life. While she still has lower white blood cell counts and mild liver enzyme elevations, researchers believe these conditions result from years of prior treatments rather than the CAR-T therapy itself.
Prof Ben Parker, a consultant rheumatologist at Manchester University NHS Foundation Trust, commented that "the prolonged response off normal therapy suggests there has been an immune reset," though the duration of this effect remains to be determined.
The research team believes early intervention with CAR-T therapy could prevent complications from years of ineffective treatments. "If we can intervene sooner, we may be able to stop the disease process, avoid organ damage, and give patients their lives back," Müller said.
The findings, published in the journal Med, represent a potential paradigm shift for treating severe autoimmune diseases, though clinical trials will be needed to establish the therapy's durability and effectiveness across different autoimmune conditions.
