Uveal Melanoma Study Shows Superior Survival Prediction
A landmark prospective, multicenter study published in Nature Communications has demonstrated that the combination of Castle Biosciences (搜索)' DecisionDx-UM (搜索) and PRAME (搜索) gene expression analysis significantly outperforms next-generation sequencing (NGS)-based gene mutation analysis in predicting survival outcomes for patients with uveal melanoma (搜索) (UM).
The study, conducted by the Collaborative Ocular Oncology Group (搜索) (COOG) and titled "Early Genetic Evolution of Driver Mutations in Uveal Melanoma (搜索)," analyzed 1,140 primary UM tumors, including 131 small, early-stage tumors. Researchers used NGS to assess seven clusters of recurrent genetic mutations and compared their predictive accuracy with the combined DecisionDx-UM (搜索) + PRAME (搜索) result.
"The DecisionDx-UM (搜索) + PRAME (搜索) combination offers the most comprehensive risk stratification available to guide more informed treatment pathway decisions for patients with UM," said J. While BAP1 (搜索), SF3B1 (搜索) and EIF1AX (搜索) gene mutations were individually associated with high, medium and low metastatic risk respectively, their ability to predict both MFS and OS became non-significant and redundant when DecisionDx-UM + PRAME were included in multivariate analysis.

Drawing on a comprehensive review of 26 published studies encompassing more than 7,500 patients, the panel used a modified Delphi process to reach unanimous agreement on nine consensus statements defining the test's role in risk stratification, sentinel lymph node biopsy (SLNB) decision making and long-term patient management.
Clinical Utility Across Disease Stages
The expert panel concluded that DecisionDx-Melanoma (搜索) demonstrates robust clinical utility and validated performance, supported by high-quality evidence showing accurate and consistent prognostic information for patients with invasive melanoma (搜索). The test can be used across a broad range of tumor stages, with panelists agreeing it should be used in earlier-stage patients to identify those who may benefit from escalated care, guide SLNB decisions in T1b–T3 tumors and inform management for SLNB-negative patients.
Integrating DecisionDx-Melanoma (搜索) results with the American Joint Committee on Cancer 8th Edition (AJCC8) staging significantly enhances prognostic precision, improving risk stratification and survival prediction when used alongside traditional clinicopathologic factors. The panel noted that patients who undergo DecisionDx-Melanoma testing demonstrate improved melanoma (搜索)-specific and overall survival compared to patients not tested, likely reflecting the test's role in supporting more personalized management strategies.
Clinical Impact and Market Adoption
DecisionDx-UM (搜索) has been widely adopted as the standard of care in the United States since 2009 and is included in the American Joint Committee on Cancer (AJCC) Staging Manual and the National Comprehensive Cancer Network (NCCN) Guidelines.
