CDC Launches Early Life Exposure Research Center to Track 9/11 Health Effects in Youngest Survivors

The youth research cohort follows a December 2022 Congressional mandate directing the CDC (搜索) to track emerging health concerns for those under 21 years old during the attacks. It also builds on a 2022 amendment to the Public Health Service Act that expanded the eligible cohort to include those within 1.5 miles of the WTC sites.
A Population Left Largely Unstudied
Approximately 40,000 children were living in Lower Manhattan when the Twin Towers collapsed, and the resulting environmental fallout was later linked to a range of acute and chronic health conditions. Despite this, no systematic study has attempted to fully understand and track the health consequences in the youngest survivors, leaving a gap in knowledge about how exposure to toxic dust and environmental contaminants affected babies in utero, infants, children, and young adults.
Writing earlier this year in an American Journal of Industrial Medicine commentary, researchers from the center noted that commemorating 25 years since the attacks allows recognition of "the successful collaboration of community, labor, academia, and government in creating a unique program to monitor and treat environmental health effects in a population of responders and civilian survivors," while also offering an opportunity to "recognize and remedy a gap" in understanding exposure in the youngest survivors.
In the 25 years since the attacks, more than 1,500 scientific publications have documented health effects among 9/11-exposed populations, according to the CDC (搜索), covering conditions of the lungs and respiratory tract, cancers, and mental health disorders. However, the commentary authors write that "only a small percentage of those publications have focused on individuals potentially exposed at a young age." Research that has focused on children found higher rates of asthma (搜索) and respiratory symptoms among those exposed to the 9/11 dust cloud in Manhattan. Other studies examined negative cardiometabolic and behavioral effects, cancer (搜索) development, and exposure to toxic forever chemicals.
Distinct Vulnerabilities in Children
Researchers have long noted that children respond differently to chemical exposures than adults. According to the CDC (搜索), children experience larger exposures relative to their body size, and their bodies are less able to eliminate chemicals. They can also face higher exposure to hazards because of crawling, putting their hands in their mouths, and faster breathing rates, said Dr. Joan Reibman, a pulmonologist and professor at NYU Grossman School of Medicine (搜索), whose research focuses on the health effects of WTC chemicals.
"We need to understand the cognitive and neurodevelopmental impacts," said Reibman, an author on the paper arguing for more research. "We need to understand cancer (搜索) incidence and the characteristics of cancer in this group."
Reibman also pointed out that most past studies have focused on male populations, leaving a "significant gap in our understanding of the impact on reproductive health." She noted that although these survivors have a longer lifespan—giving more time for disease to develop—few have been tracked in formal studies.
Enrollment Barriers and Program Turmoil
Both the WTC Health Program and the New York City WTC Health Registry were open to those exposed at a young age, including in utero, Reibman said. However, relatively few individuals meet these criteria: only about 2,400 survivors are enrolled in the CDC (搜索) program, while 3,200 are enrolled in the NYC program. This prompted a "concerted effort" to enroll more people, ultimately leading to the 2022 legislative expansion.
Reibman acknowledged the difficulties in assembling a study large enough to link certain exposures to health outcomes. The WTC Youth Research Coordinating Center "will have to overcome significant barriers to recruitment, which will require the involvement of community groups and novel recruitment techniques," she said. An interdisciplinary team of biostatisticians, epidemiologists, and disease specialists will also be needed to determine the impact of exposures on disease.
Captain Matt Karwowski of the Commissioned Corps of the US Public Health Service, chief of the research and evaluation branch at the WTC Health Program, told Health Policy Watch that enrollment would begin after the coordinating center is fully established.
The announcement comes after a year of turmoil for the CDC (搜索), with widespread reductions in workforce across federal agencies. In February, Department of Government Efficiency-related efforts led to a 20% reduction in WTC program staff, prompting outcry from New York lawmakers and concerns that certifications to allow treatment of new conditions and cancers would be delayed.
Decades of Research and Clinical Infrastructure
The 9/11 attacks released unprecedented quantities of complex mixtures of chemicals into the environment. Years of advocacy by affected communities and academics resulted in passage of the James Zadroga 9/11 Health and Compensation Act, the basis for the WTC Health Program. Created in 2011 by the CDC (搜索)'s National Institute for Occupational Safety and Health, the program provides medical monitoring and treatment of certain WTC-related respiratory diseases, cancers, and mental health conditions (搜索), and funds research into health conditions associated with exposure to dust, debris, and smoke from the attacks in New York, Shanksville, and the Pentagon.
The federal program provides eligible responders and survivors with medical monitoring, screening, and treatment for conditions including respiratory diseases such as asthma (搜索), COPD (搜索), and sleep apnea (搜索), mental health conditions (搜索), and more than 50 types of cancer (搜索).
Separately, University of Rochester Medicine (搜索) has contributed to understanding WTC exposures through environmental health research, community education, and long-term occupational health care. In the months following the attacks, researchers in the URochester Medicine Environmental Health Sciences Center—including Günter Oberdörster, PhD, Jacob (Jack) Finkelstein, PhD, and Mark Utell, MD—worked with colleagues at New York University and elsewhere to investigate the potential health effects of WTC dust, supported by the National Institute of Environmental Health Sciences. The team analyzed dust collected from apartment buildings, parked cars, and streets in Lower Manhattan, characterizing its chemical, physical, and microscopic properties and studying its effects on lung cells, including whether exposure could trigger changes associated with inflammation or increased susceptibility to respiratory infections.
While initial studies were unclear about immediate health risk, subsequent research showed that WTC dust and smoke were complex mixtures containing potentially hazardous substances, and that the intensity and duration of exposure were important factors in determining health risks. Over the ensuing decades, studies of responders and survivors documented persistent respiratory disease (搜索), mental health conditions (搜索), and numerous cancers associated with 9/11 exposure. Newly released records have provided additional insight into environmental conditions following the attacks, including evidence that asbestos (搜索) and other contaminants persisted in some areas.
"The World Trade Center research is part of a long legacy of environmental health research at URochester, where scientists have worked for decades to understand how environmental exposures—including air pollution and other airborne toxicants—affect human health," said Paige Lawrence, PhD, chair of the Department of Environmental Medicine and Public Health Sciences. "One of the challenges in environmental health is that the effects of an exposure are not always immediate or straightforward. This is a powerful example of why sustaining research matters and how environmental health research connects laboratory science, population health, clinical care, and community needs."
Through Finger Lakes Occupational Health Services, URochester Medicine became part of a nationwide network providing medical evaluation, monitoring, and treatment to WTC responders living in the Finger Lakes and Western New York. FLOHS, part of the New York State Occupational Health Network and based in the Department of Environmental Medicine and Public Health Sciences, remains affiliated with the program's Nationwide Provider Network, providing annual medical surveillance, clinical referrals to specialists, and education about occupational and environmental exposures. Those receiving services include firefighters, state troopers, Red Cross volunteers, and others who participated in rescue and recovery efforts.
The lessons of 9/11, URochester researchers note, reinforce that environmental health questions rarely have simple or immediate answers, and that understanding the health consequences of complex exposures can require decades of research, population monitoring, clinical care, and engagement with affected communities.
