Comparable Efficacy Across Cancer Types
A study published in AIDS by Anna Coghill, PhD, MPH, of Moffitt Cancer Center compared checkpoint inhibitor outcomes between 24 HIV (搜索)-positive and 24 HIV-negative cancer (搜索) patients. The overall response rate was 29% for HIV-positive patients versus 38% for HIV-negative patients, though this difference was not statistically significant. Overall survival rates were nearly identical at 63% and 67%, respectively, with no significant differences in adverse event frequencies.
"These findings suggest that HIV (搜索) status alone should not preclude use of immune checkpoint inhibitor therapy to improve prognosis among people with HIV and cancer (搜索)," the study authors concluded.
Real-World Evidence in Skin Cancers
Research published in The Oncologist examined outcomes among 54 HIV (搜索)-positive patients with aggressive skin cancers treated with checkpoint inhibitors. Response rates varied by cancer (搜索) type: 43% for melanoma (搜索) patients, 58% for cutaneous squamous cell carcinoma (搜索), and 75% for Kaposi sarcoma (搜索). Immune-related adverse events occurred in 43% of patients overall, increasing to 65% among the 17 patients treated with combination Opdivo (nivolumab) and Yervoy (ipilimumab) therapy.
The study authors concluded that findings "support managing well-controlled people with HIV (搜索) similarly to the general population and highlight the importance of including this population across all settings of skin cancer (搜索) trials."
Systematic Review Confirms Lung Cancer Benefits
A systematic review published in MDPI Pharmaceuticals analyzed outcomes for HIV (搜索)-positive patients with non-small-cell lung cancer (搜索) (NSCLC) treated with PD-1 (搜索) or PD-L1 (搜索) checkpoint inhibitors. The analysis included six studies encompassing 762 patients, most of whom were on antiretroviral therapy with viral suppression. Patients received treatment with Keytruda (pembrolizumab), Opdivo, Tecentriq (atezolizumab), or Imfinzi (durvalumab).
Despite wide variation in outcomes across studies, researchers concluded that efficacy results were "encouraging" and aligned with general lung cancer (搜索) population outcomes. "These data indicate that, when appropriately selected, people with HIV (搜索) with NSCLC may derive comparable survival benefits from immune checkpoint inhibitors as their HIV-negative counterparts," the authors wrote.
HIV Control Remains Intact
A separate review in HIV (搜索) Medicine identified five relevant NSCLC studies, confirming that response rates and survival outcomes for HIV-positive patients were similar to HIV-negative individuals. Importantly, no significant effects of checkpoint inhibitors on HIV viral load or CD4 count were reported.
This finding was reinforced by the case of Loreen Willenberg, an elite HIV (搜索) controller who maintained viral suppression even after receiving Keytruda treatment for lung and brain tumors, with no evidence of intact HIV remaining detectable.
Potential HIV Cure Applications
Beyond cancer (搜索) treatment, checkpoint inhibitors may contribute to HIV (搜索) cure research. The PD-1 (搜索) receptor is heavily expressed on exhausted T cells that have lost functional ability and on CD4 helper T cells harboring hidden HIV. In one small study, six out of 11 people receiving the investigational PD-1 (搜索) inhibitor budigalimab experienced slower viral rebound during analytical treatment interruption, with two maintaining viral suppression for more than a year.
