Cognito Therapeutics (搜索) and the West Virginia University Rockefeller Neuroscience Institute (搜索) (RNI) have announced the launch of the first Brain Health Collaboratory, a pioneering initiative designed to accelerate the discovery, validation, and real-world delivery of non-drug neurotherapies. The collaboration represents a new model that merges clinical care, AI-powered research, and continuous data generation into a single learning healthcare ecosystem.
Spectris™ Device at the Center of Innovation
At the core of this initiative is Spectris™ (搜索), Cognito's investigational, non-invasive neuroprotective device designed for at-home use. The headset delivers acoustic and light-based stimulation at a 40 hertz frequency for one hour per day. Since earning FDA Breakthrough designation in 2020, Cognito has completed a six-month early feasibility study with 76 patients and a subsequent voluntary 12-month extension to study longer-term outcomes.
The company has published data suggesting their "neuroprotective" approach can help preserve cognition, brain function, and brain volume while reducing health system costs. "After 60 days of stimulation at 40 hertz, you can see what's happening by looking at people's spinal fluid," explained Christian Howell, CEO of Cognito Therapeutics (搜索). "We're seeing significant protein expressions in synapses (搜索), myelin and cognitive resilience proteins. At 90 to 120 days under MRI, you can see not just myelin preservation, but those proteins actually having a restorative effect, building synaptic plasticity and improving glymphatic function (搜索)."
Cognito's pivotal study is now fully enrolled with 673 patients and anticipated completion in spring 2026. Spectris™ (搜索) has shown early promise in preserving brain structure and function in Alzheimer's and may hold potential across a wide range of conditions, including Parkinson's disease (搜索), multiple sclerosis (搜索), traumatic brain injury (搜索), stroke (搜索), and addiction (搜索).
Comprehensive Healthcare Ecosystem Integration
The Brain Health Collaboratory builds upon RNI's infrastructure that integrates comprehensive multidisciplinary patient care with robust translational research capabilities. RNI operates 47 practice locations with telehealth capabilities, caring for approximately 300,000 patients across rural West Virginia, Southwestern Pennsylvania, Eastern Ohio, and Western Maryland.
"Our ecosystem has 330 different physicians and scientists in the neuroscience institute and about 2,400 team members," said Dr. Ali Rezai, executive chair of the Rockefeller Neuroscience Institute. "All of them work on the same record system, with the same imaging standards for MRIs, EEGs, EMGs. The same team analyzes data. So we found this a very unique opportunity to study an in-home neurotherapeutic product, follow it longitudinally and advance the science behind it."
RNI cares for approximately 10,000 patients annually with Alzheimer's disease (搜索) and 9,000 with Parkinson's disease (搜索). The institute also operates as a payer-provider with its own insurance company, enabling comprehensive cost-effectiveness analysis.
AI-Powered Research and Data Analytics
The Collaboratory leverages extensive AI capabilities integrated throughout RNI's operations. "We've been following 6,000 patients in our ecosystem for the past five years, looking at people with severe addiction (搜索) or Alzheimer's, Parkinson's, vascular dementias," Rezai explained. The system incorporates digital disease management, longitudinal tracking across wearables, sleep tracking, heart rate variability, electronic medical record data, telehealth visits, medications, and patient survey applications.
"All that data goes into a large, continuous backbone of AI," Rezai noted. "The key thing is context with the data, linking to whether people are getting worse or getting better, or thriving. That context is absolutely mission critical. Otherwise, the AI systems are handicapped because they don't have the clinical context."
Addressing Caregiver Burden and Health Economics
Given the in-home nature of the Spectris treatment and the acute impact of Alzheimer's disease (搜索), caregiver engagement represents a critical component of the initiative. "The family member burden is enormous in terms taking care of the patient and the entire family," explained Rezai. "The emotional impact and financial impact on these people is very significant. In our system we have a caregiver program to address emotional and physical health, with counselors and other support provided free of charge."
The collaboration aims to demonstrate community-level impact of earlier and more effective therapy, particularly in rural settings. "This is a big opportunity to show that we can help save people money, save visits, help caretakers not have to leave their jobs, take time off work, stay in the workforce," Rezai said.
Future Expansion and Commercialization Strategy
While their HOPE pivotal trial aims to demonstrate clinical effectiveness to meet FDA expectations, Howell emphasized that the initiative extends beyond clinical evidence to address cost effectiveness and health economics needed for CMS coverage determination. "The evidentiary burden for drugs is in the pre-market, and the evidentiary burden for devices is in the post-market. But the only way you can shift that burden in the post market to start with robust premarket evidence generation."
The center is co-funded by Cognito and RNI, with plans to draw upon external support including government funding, NIH grants, foundations, and donors as data demonstrates the therapy's ability to change the course of Alzheimer's disease (搜索). Howell expects Cognito will forge "only three or four of these types of Collaboratories" given the high bar for finding aligned health system leadership and the depth of involvement required.
The Collaboratory supports full-stack research and development from pre-clinical to clinical trials, health economics and outcomes research (HEOR), real-world data (RWD) analytics, and commercialization planning. "This is a rare inflection point where science, technology, and healthcare systems converge to transform how we treat brain disease," said Howell. "Spectris has the potential to change millions of lives, but realizing that promise requires bold, global collaboration and entirely new models of evidence generation."
