A comprehensive analysis of major clinical trials has revealed that continuous positive airway pressure (搜索) (CPAP (搜索)) therapy produces markedly different cardiovascular outcomes depending on a patient's sleep apnea (搜索) risk profile, with potentially harmful effects in some populations previously considered candidates for treatment.
The post-hoc analysis, published in the European Heart Journal, examined data from more than 3,500 participants across three randomized trials to determine whether CPAP (搜索)'s cardiovascular benefits occur preferentially in patients with high-risk obstructive sleep apnea (搜索) (OSA (搜索)). The research was led by Ali Azarbarzin, a sleep medicine investigator at Brigham and Women's Hospital and Harvard Medical School.
Divergent Outcomes Based on Risk Stratification
The study found that patients with high-risk OSA (搜索)—characterized by dramatic drops in blood oxygen levels or large surges in heart rate—experienced a 17% reduction in major adverse cardiovascular and cerebrovascular events (MACCE) when treated with CPAP (搜索) compared to usual care. Overall, 16.6% of CPAP-treated participants and 16.3% of usual care participants reached the MACCE endpoint.
However, the analysis revealed concerning findings for patients with low-risk sleep apnea (搜索). Among individuals without high-risk OSA (搜索) markers who did not experience excessive daytime sleepiness, CPAP (搜索) therapy appeared to increase the risk of serious heart problems by 30%.
"Our findings suggest that CPAP (搜索) may offer long-term cardiovascular benefit in people with high-risk obstructive sleep apnea (搜索), but may have unintended harmful effects in those without high-risk OSA (搜索)," Azarbarzin said in a news release.
Mechanism of Action Varies by Patient Profile
The differential effects appear to stem from distinct physiological mechanisms. In high-risk patients, CPAP (搜索) likely provides cardiovascular protection by preventing severe oxygen desaturation and calming the overactive sympathetic nervous system (搜索) during sleep—both factors linked to heart disease (搜索) development.
"For people with high-risk OSA (搜索), CPAP (搜索) likely helps by preventing low oxygen levels and calming the overactive sympathetic nervous system (搜索) during sleep," Azarbarzin explained. "But in people without these high-risk markers, who are already at very low cardiovascular risk, CPAP seems to have downsides."
The researchers hypothesize that in low-risk patients, the positive pressure delivered by CPAP (搜索) machines may stretch the lungs in ways that stress the cardiovascular system. Additionally, CPAP could disrupt sleep quality in some individuals, and sleep disruption itself represents a cardiovascular risk factor.
Clinical Implications for Treatment Guidelines
The findings have significant implications for current sleep apnea (搜索) management approaches. The research showed particularly strong differential effects in patients without excessive sleepiness or elevated blood pressure, with interaction hazard ratios of 0.59 and 0.54, respectively.
"For clinicians and policymakers, this means we may need to rethink current guidelines for treating nonsleepy patients and consider focusing treatment for cardiovascular risk reduction on those who stand to gain the most," Azarbarzin stated. "We also need to be cautious about using CPAP (搜索) in people who don't have high-risk markers, as they may not benefit and could even be harmed."
Study Limitations and Future Research Needs
The researchers acknowledged important limitations in their analysis. The CPAP (搜索)-related harm was observed specifically in non-sleepy patients with existing heart disease (搜索), based on the design of the trials analyzed. Whether these findings apply to other patient populations remains unknown and requires further investigation.
"Instead of treating everyone the same, we should consider whether someone has high-risk features," Azarbarzin said. "These are the people who seem most likely to benefit from CPAP (搜索)."
The study represents a significant shift toward personalized medicine in sleep apnea (搜索) treatment, moving away from universal CPAP (搜索) prescription toward risk-stratified therapeutic approaches.
