CTx001 Gene Therapy Shows Early Tolerability and Slowed Lesion Progression in Geographic Atrophy

Early imaging from the first cohort showed slowed lesion progression in treated eyes and stabilized microperimetry, even with short follow-ups of about one month.
CTx001, an investigational subretinal gene therapy developed by Complement Therapeutics (搜索), was well tolerated at a low dose and demonstrated early signals of slowed lesion progression in patients with geographic atrophy (搜索), according to initial findings from the phase 1/2 Opti-GAIN study presented at Clinical Trials at the Summit in Las Vegas.
Arshad Khanani, MD, MA, FASRS, of Sierra Eye Associates in Reno, Nevada, presented data from the first cohort of patients enrolled in the trial. The primary endpoints are safety and tolerability of CTx001 as well as change in ellipsoid zone attenuation at 52 weeks compared with baseline.
A Novel Structure-Function Approach
The Opti-GAIN study implemented one of the first prospective, ellipsoid zone-based perilesional microperimetry approaches, designed to connect structural and functional outcomes in geographic atrophy (搜索). The trial includes a 6-month run-in period followed by prospective evaluation of the relationship between structure and function using ellipsoid zone image-guided microperimetry.
"This trial is powered to show a change in ellipsoid zone, and it involves a 6-month run-in followed by looking prospectively at the connection between structure and function using ellipsoid zone image-guided microperimetry," Khanani told Healio.
Early Efficacy and Safety Signals
Even with short follow-ups of approximately one month after a low dose of CTx001, imaging for one patient demonstrated slowed lesion progression in the study eye and lesion growth in the fellow eye. Eyes in the first cohort also exhibited stabilization of microperimetry, according to Khanani.
"To date, CTx001 has been well tolerated in the low dose of 5E10, and other doses are currently being evaluated," Khanani said.
The Importance of Functional Endpoints
Roger A. Goldberg, MD, MBA, a retina specialist at Bay Area Retina Associates, discussed the significance of the study's design, emphasizing the value of testing at the margins of geographic atrophy (搜索) lesions.
"One of the things we've learned is you really need to be able to test right at the edges of the geographic atrophy (搜索) because that's where you're going to be able to detect a difference," Goldberg said.
"The real important thing here is that we're going to be able to have both a functional and an anatomic endpoint that are tied to each other and that are relevant for patients with geographic atrophy (搜索)," he added.
The findings highlight an evolving approach to geographic atrophy (搜索) clinical development, in which functional assessments are paired with anatomic measures to better capture treatment effects in a disease characterized by progressive retinal degeneration.
