Danicamtiv Shows Promise in Phase 2a Trial for Genetic Dilated Cardiomyopathy

A novel cardiac myosin (搜索) activator called danicamtiv has shown promising results in improving heart function in patients with genetic dilated cardiomyopathy (搜索), according to Phase 2a trial data presented at the Heart Failure (搜索) Society of America Annual Scientific Meeting and published simultaneously in the Journal of the American College of Cardiology.
The single-arm, open-label trial enrolled 41 patients with dilated cardiomyopathy (搜索) (mean age 50 years, 37% women), including 12 patients with MYH7 (搜索) gene variants, 14 with TTN (搜索) gene variants, and 15 with other causes of the disease. Patients received danicamtiv 25 mg twice daily for 5-8 days, followed by either 10 mg or 50 mg twice daily for another 5-8 days, with dosing determined by systolic ejection time response.
Significant Functional Improvements
The trial demonstrated statistically significant improvements in cardiac function, particularly in patients with genetic variants. At the end of the second treatment period, mean left ventricular ejection fraction (LVEF) improved by 8.8 percentage points in the MYH7 (搜索) group (P = 0.001), 5.9 percentage points in the TTN (搜索) group (P = 0.005), and 4.4 percentage points in the other causes group.
Left ventricular global longitudinal strain also showed meaningful improvements, declining by 2.1 percentage points in the MYH7 (搜索) group (95% CI, -3.26 to -0.84), 1.2 percentage points in the TTN (搜索) group (95% CI, -2.38 to -0.04), and 1.4 percentage points in the other causes group (95% CI, -2.71 to -0.04).
The left atrial function index increased significantly in the MYH7 (搜索) cohort by 11.1 points (95% CI, 4.92-17.37; P = 0.006) and by 6.8 points in the TTN (搜索) cohort (95% CI, -0.11 to 13.8) at the end of the second treatment period.
Novel Mechanism of Action
Danicamtiv works through a unique mechanism that distinguishes it from traditional heart failure (搜索) treatments. "Danicamtiv directly activates cardiac myosin (搜索) and essentially increases the force of contraction of the heart without increasing the concentration of calcium, which is an important distinction from inotropes, which use that as their method of increasing contractility," explained Neal K. Lakdawala, MD, director of the Cardiovascular Genetics Clinic at Brigham and Women's Hospital.
This calcium-independent approach is significant because traditional inotropes that increase calcium concentrations carry an increased risk of arrhythmias, a problem that danicamtiv appears to avoid.
Safety Profile
The drug demonstrated a favorable safety profile throughout the study period. Adverse events occurred in 63.4% of patients, with treatment-emergent adverse events in 53.7%, all of which were mild or moderate. Importantly, there were no serious adverse events, deaths, or drug-related adverse events leading to discontinuation during the study period.
Precision Medicine Approach
The trial was specifically designed to assess whether danicamtiv could improve contractility in patients with dilated cardiomyopathy (搜索) caused by mutations in genes encoding proteins that the drug activates. "We wanted to learn whether a precision approach was possible," Lakdawala said.
The results support this precision medicine strategy, with patients carrying MYH7 (搜索) variants showing the most robust response to treatment. "It did so without significant hazard in the form of troponin release, especially in patients with myosin-related dilated cardiomyopathy (搜索)," Lakdawala noted.
Clinical Significance
Dilated cardiomyopathy (搜索) affects approximately one in every 250 people and represents a significant unmet medical need. "Dilated cardiomyopathy is a devastating and progressive disease with no available treatments that target its underlying causes," Lakdawala stated. "Left untreated, dilated cardiomyopathy presents a significant risk of disease progression to advanced stages of heart failure (搜索)."
The current findings represent a departure from the traditional approach to treating dilated cardiomyopathy (搜索). "For too long, we have treated this disease as a homogenous entity when it is not," Lakdawala explained. "We know that genetic causes underlie dilated cardiomyopathy in approximately 30% of cases. Patients can have a very different natural history and clinical manifestations based on the underlying cause."
Future Development
Based on these positive results, Kardigan (搜索) plans to initiate the Phase 3 KINSHIP-DCM trial later this year. "We hope to learn whether or not these changes in contractile function translate into improved markers of cardiac performance and functional capacity, whether they are sustained and how they compare to a placebo-treated group," Lakdawala said.
