Biopharmaceutical company Dompé (搜索) has enrolled the first patients in its Phase 3 study of isocyclosporin (搜索) for the treatment of atopic keratoconjunctivitis (搜索) (AKC), marking a significant milestone in addressing a severe chronic inflammatory eye disease with no FDA-approved treatment options. The multicenter, randomized, double-masked study will compare isocyclosporin against a vehicle control in improving ocular signs and symptoms after six weeks of treatment.
Novel Dual-Mechanism Approach
Isocyclosporin (搜索) represents a breakthrough therapeutic approach as a dual inhibitor of transient receptor potential (TRP) cation channel, mucolipin subfamily, member 2 (TRPML2 (搜索)), and calcineurin (搜索). Both pathways serve as key regulators of immune and inflammatory response in chronic inflammatory eye diseases. TRPML2 acts as a key mediator of immune cell activation, while calcineurin regulates T-cell mediated immune responses.

Addressing Critical Treatment Gap
AKC is a severe chronic inflammatory eye disease that frequently occurs alongside atopic conditions like eczema (搜索) or asthma (搜索). The condition results from an overactive, chronic inflammatory reaction in the eyes, where the body's immune system becomes overly sensitive to everyday allergens such as dust or pollen, leading to long-term inflammation and irritation.
Currently, patients with AKC face limited treatment options, relying on therapies such as topical antihistamines, corticosteroids, immunomodulators, and emerging biologics. These existing treatments are often intended for short-term use due to severe side effects, particularly with corticosteroids that carry risks of toxic side effects like glaucoma (搜索) and cataracts (搜索).
"Today, patients with AKC are faced with limited treatment options often intended for short-term use due to severe side effects," explained Ahmed Enayetallah, Chief Development Officer at Dompé (搜索). "The inability to achieve long-term disease control diminishes day-to-day quality of life and exposes patients to potential vision loss from complications like scarring and ulceration."
Clinical Development Progress
The Phase 3 advancement follows encouraging results from a pivotal Phase 2 study (ISAAK, EU_CT#: 2023-508907-19-00), where isocyclosporin (搜索) demonstrated statistically significant improvements in measures of AKC-associated signs and symptoms after four weeks of treatment compared to vehicle control. Notably, the study reported no discontinuations due to isocyclosporin-related adverse events and no difference in post-installation comfort scores compared with vehicle control.
Disease Impact and Diagnosis Challenges
AKC presents with hallmark symptoms including severe itching, redness, and eye discomfort, while also making the eyes more vulnerable to infections and chronic pain. The persistent and debilitating symptoms can significantly disrupt daily activities and lead to emotional distress, reducing overall quality of life. If not properly managed or treated, AKC can cause scarring or thinning of the cornea, potentially leading to blurry vision or permanent vision loss.
Limited disease awareness often leads to delayed or missed diagnosis, with AKC frequently misdiagnosed as other forms of allergic eye disease or less severe conditions affecting the eye surface. This diagnostic challenge can result in delayed or improper treatment, further emphasizing the need for effective therapeutic options.
Therapeutic Design Features
Isocyclosporin (搜索) is specifically designed for enhanced ocular solubility to improve bioavailability, distribution, and tolerability. The therapy aims to provide long-term treatment control of the inflammatory reaction in AKC, reducing symptoms and complications while sparing topical steroid use and its associated risks.
