Spencer Laird, a 30-year-old mechanic from South Carolina, experienced one of the most striking responses in the trial. After his colorectal cancer (搜索) metastasized to his lungs with 13 tumors—including three the size of golf balls—doctors initially gave him two to three years to live with conventional chemotherapy (搜索) and radiation.
Novel Treatment Approach Shows Dramatic Results
Instead of pursuing standard chemotherapy (搜索), Laird enrolled as the second patient in Duke's clinical trial testing Botensilimab and Balstilimab as first-line immunotherapy (搜索). The trial, led by gastrointestinal oncologist Dr. Nicholas DeVito, represents a significant departure from current treatment protocols for microsatellite stable colorectal cancer (搜索).
"In a nutshell, this was a diagnosis that would probably have shortened his life to five years or less, at best," DeVito said. "And now we're thinking 'Well, is he cured? How long does this remission last? Is it permanent? How does this cancer stay away?' We're really drifting into uncharted territory."
After one year of treatment, Laird's response has been remarkable. His 13 lung tumors have been reduced to just 2-3 small spots, with the largest measuring only 0.6 centimeters. DeVito noted that the remaining areas may no longer be cancerous and could represent scar tissue.
Addressing Unmet Medical Need
Microsatellite stable colorectal cancer (搜索), the most common form of the disease, currently has no approved immunotherapies. Chemotherapy (搜索) remains the primary treatment option when the disease spreads, but it typically controls disease for only six months in 80-90% of patients, according to Dr. Michael James Overman, an oncologist at MD Anderson Cancer Center who was not involved in the trial.

"While chemotherapy has high response rates, it doesn't work forever and it doesn't even work for a very long period of time," DeVito explained. "Immunotherapy can work for a very long period of time and even cure some people of stage IV cancer."

The study enrolled 15 patients in 14 months, reaching its two-year enrollment goal ahead of schedule. Patients receive immunotherapy (搜索) infusions every two weeks, with scans taken every six to eight weeks to monitor disease progression. Beyond imaging improvements, Laird's CEA (搜索) tumor marker—an imperfect but useful blood marker for monitoring colon cancer (搜索) treatment—has plummeted. "That along with the scans and how he feels, all support this idea that he has a long and enduring response to immunotherapy (搜索), which is a first," DeVito said.
Laird experienced some initial side effects including rashes, headaches, nausea, and dizziness during early treatment, but these resolved by late spring 2025. Since then, treatment has been "smooth sailing" with only occasional stomach troubles.
Future Research Directions
DeVito will present interim data from the clinical trial at the American Association for Cancer Research (搜索) national conference in April. The next steps include expanding the trial and conducting biomarker-driven investigations to better understand which patients may benefit most from immunotherapy (搜索).
"The real beauty of this study is that we're only treating patients with immunotherapy (搜索). Therefore, you get an idea of how the cancer changes in response to only that immunotherapy," DeVito said. "You're not throwing the kitchen sink at someone—chemo, immunotherapy, targeted therapy—and then you say, 'I don't know what did what.'"
Overman emphasized that while the trial is novel, significant research remains before early immunotherapy (搜索) can be considered standard care. Future studies should investigate biomarkers that predict immunotherapy response and continue refining patient selection criteria.
Clinical Impact and Broader Implications
The trial addresses a critical gap in treatment options for patients who prefer to avoid chemotherapy (搜索)'s side effects while maintaining quality of life. "We don't have a lot of non-chemo approaches. That's the real novel aspect here," Overman said. "This here is proof of concept. You can do it front line, and there's definitely some activity."
DeVito believes this approach may extend beyond colorectal cancer (搜索) to other cancer types, advocating for studies that "push the envelope" by testing novel agents earlier in therapy. "We should be pushing the limits and saying what we're doing for patients is not good enough," he said.
For Laird, the results have transformed his prognosis from terminal to manageable. While not cancer-free, his disease is no longer considered terminal, allowing him to focus on his 6-year-old daughter and future plans. "I can't say I'm cancer-free, but I feel like I am," Laird said.
