The study examined duodenal mucosal resurfacing in patients who had achieved substantial weight loss using Eli Lilly's tirzepatide, sold as Zepbound for obesity (搜索) and Mounjaro for diabetes. All trial participants had lost at least 15% of their body weight, approximately 40 pounds, before stopping the medication.
Clinical Trial Results
Results from the first 45 participants in the ongoing 300-patient trial revealed striking differences between treatment groups. Patients who underwent a sham procedure regained 40% more weight than those receiving the actual treatment at six months post-discontinuation.
Most notably, patients who had the most tissue resurfaced maintained over 80% of their weight loss, regaining only about 7 pounds on average. This represented roughly half the weight regained by the control group during the same period.
"What's particularly encouraging is that the benefit appears to increase over time rather than fade, and that it behaves like a drug in terms of dose response," said study leader Dr. Shelby Sullivan of the Dartmouth Geisel School of Medicine (搜索). The weight difference between groups appeared to widen from one to six months post-intervention.
Addressing an Unmet Medical Need
The procedure targets a significant clinical challenge facing patients using GLP-1 medications. "As effective as GLP-1 medications are, many people stop taking them because of cost, side effects or simply not wanting to take a drug long-term," Dr. Sullivan explained. "But, if they stop these medications, weight regain occurs in the vast majority of patients, and the metabolic benefits are lost."
Recent studies indicate that patients who discontinue GLP-1 drugs (搜索) after achieving weight loss results regain nearly two-thirds of their lost pounds, highlighting the need for sustainable maintenance strategies.
Mechanism of Action
Duodenal mucosal resurfacing uses targeted heat to destroy the interior surface of the duodenum (搜索), the upper part of the small intestine. Over time, high-fat, high-sugar diets can lead to changes in the duodenal lining, ultimately rewiring how the gut responds to food and affecting hormone production, leading to insulin resistance (搜索) and metabolic diseases (搜索).
The procedure allows for regeneration of healthier tissue and appears to "reset" individuals' metabolism to their new post-GLP-1 weight. The targeted tissues are typically involved in producing hormones that are mimicked by GLP-1 drugs (搜索).
Procedure Characteristics
The minimally invasive procedure offers practical advantages for patients. "You can be back to your daily routine in about a day. Participants could not tell if they had the sham or real procedure because there are not a lot of symptoms after the procedure," Dr. Sullivan noted.
The procedure is currently approved for patients with type 2 diabetes (搜索) in Europe but remains experimental in the United States.
Future Development
Trial sponsor Fractyl Health expects to release six-month data on the entire study population and apply for regulatory approval by the end of 2026. The researchers characterized their findings as indicating that this minimally invasive procedure may provide lasting weight-loss maintenance.
"Finding a treatment that allows patients to stop these medications without weight regain or loss of metabolic benefit is a huge unmet need," Dr. Sullivan emphasized, highlighting the potential clinical significance of the approach.
