ESMO Commits €1 Million to Fund Academic Cancer Trials Targeting Treatment De-escalation and Care Optimization

The European Society for Medical Oncology (ESMO (搜索)) has launched a Clinical Trials Funding Initiative, committing €1 million to support independent academic clinical research aimed at optimizing cancer (搜索) care, reducing treatment burden and improving patient outcomes worldwide. The program was announced on September 10, 2026, and forms part of the ESMO Roadmap 2030.
A Focus on Optimizing Existing Treatments
Rather than concentrating primarily on the development of new anticancer medicines, the initiative is designed to investigate how existing treatments and care pathways can be used more effectively and efficiently. Research areas may include treatment duration, treatment sequencing, treatment de-escalation, streamlined treatment delivery and alternative models of care. The stated objective is to reduce unnecessary treatment and its associated toxicity and burden while maintaining clinical effectiveness and improving access to optimal cancer (搜索) care.
The program will support pragmatic interventional studies with international relevance, particularly research addressing questions that may carry major implications for patients and healthcare systems but are often not priorities for commercially sponsored trials.
"Many important questions in oncology relate not to the development of new medicines, but to the optimization of existing treatments and care pathways," said Fabrice André, MD, PhD, ESMO (搜索) President. He added that the initiative is intended to strengthen support for academic research, "recognizing the essential role of independent clinical research in addressing unmet needs in oncology."
In a LinkedIn post, André also highlighted the global-access implications of the funding: "ESMO (搜索) launches an initiative to fund academic clinical trials. The funding will be used to expand existing trials in new areas of the world. The trials will be selected to reduce treatment burden while keeping efficacy, and thereby opening access to optimal care to more patients."
Eligibility and Application Requirements
Eligible applicants include academic institutions, cancer (搜索) centres, hospitals and cooperative research groups developing interventional clinical trials aligned with the program's objectives. Studies may already be recruiting, be approved but not yet recruiting, or be awaiting regulatory and institutional approvals at the time of application.
Applications will be evaluated on scientific quality, practice-changing potential, patient impact, sustainability, feasibility and global relevance, with particular emphasis on projects capable of generating evidence that can inform routine clinical practice across diverse healthcare settings. The program also includes requirements relating to governance, quality assurance, reporting, public registration and data-sharing, reflecting what ESMO (搜索) describes as its commitment to transparency and research integrity.
Consistent with its focus on patient-centred research, ESMO (搜索) expects applicants to describe how patients have been involved in study design and dissemination plans. The application deadline is November 1, 2026, with the Call for Applications, Applicant Guidance Document and Application Form available through ESMO's Clinical Trials Funding Initiative webpage.
Questions Industry May Not Address
According to ESMO (搜索), independent academic trials can play a particularly important role in answering questions where there may be limited commercial incentive to conduct large prospective studies. These include whether treatments can safely be given for shorter periods, at lower intensity or with fewer interventions; whether different sequencing strategies produce equivalent or better outcomes; and whether changes in treatment delivery can improve quality of life and access while preserving efficacy. ESMO describes such independent academic research as an important component of improving both patient outcomes and the sustainability of cancer (搜索) care. Oncology Community Response
The announcement drew positive reactions from oncologists who have advocated for greater investment in treatment optimization and de-escalation studies.
