A new experimental drug for narcolepsy type 1 (搜索) (NT1 (搜索)) has shown promising results in improving cognitive function, according to a secondary analysis of a phase 2 randomized clinical trial published in JAMA Neurology. The study found that oveporexton, developed by Takeda Pharmaceuticals (搜索), not only addresses the hallmark symptoms of NT1 but also significantly enhances memory, attention span, and executive functioning.
Addressing an Overlooked Burden
Narcolepsy type 1 (搜索) emerges when neurons in the hypothalamus that produce orexin (搜索) taper off, disrupting the brain's normal regulation of sleep, muscle tone, attention, arousal, and reward processing. While clinicians have traditionally focused on treating excessive daytime sleepiness and cataplexy, patients consistently report a different challenge: cognitive difficulties.
Multiple surveys indicate that up to 90% of adults with NT1 (搜索) describe notable cognitive difficulties, including struggles to think clearly, learn new things, concentrate, and remember details, even when receiving treatment for other symptoms.
Novel Mechanism of Action
Oveporexton belongs to a new class of selective orexin receptor 2 (搜索) (OX2R (搜索)) agonists designed to directly replace lost orexin (搜索) signaling rather than compensate for it. This approach represents a fundamental shift from existing treatments that primarily manage symptoms rather than address the underlying biological cause.
An earlier analysis of the same clinical trial demonstrated robust improvements in sleepiness, cataplexy, and quality of life. The current follow-up study explored whether restoring orexin (搜索) signaling would also improve cognitive function.
Trial Design and Results
The TAK-861-2001 phase 2, parallel-group, double-blind, placebo-controlled study analyzed 112 adults between ages 18 and 70 who received either placebo or various dose regimens of oveporexton over eight weeks. The international trial ran from January 2023 to December 2023 with a 4-week follow-up period.
Participants were randomized 1:1:1:1:1 to receive twice-daily oral oveporexton or matched placebo, dosed 3 hours apart, in dose groups of 0.5/0.5 mg, 2/2 mg, 2/5 mg, 7 mg/placebo, or placebo/placebo. All narcolepsy medications were discontinued before the trial, and cognitive testing occurred one hour after the morning dose.
The study population had a mean age of 34 years and was 52% female. Researchers assessed cognitive symptoms using multiple neuropsychological measures: the Continuous Paired Associate Learning (CPAL) test for memory, the Psychomotor Vigilance Task (PVT) for attention, and the One Back (ONB) test and International Digit Symbol Substitution Test–symbols (IDSST-s) for executive function.
Significant Cognitive Improvements
After eight weeks, participants receiving oveporexton showed dramatic improvement on all four cognitive measures across nearly every dose compared to those taking placebo. For attention, placebo patients actually slowed over time, while treated patients improved. For memory, oveporexton users made far fewer errors, with medium-to-large effects across doses.
Specifically, oveporexton enhanced ONB processing speed, increased accurate answers on the IDSST, and decreased PVT lapses and CPAL mistakes across dosages when compared to placebo. Executive functions also improved, especially at higher doses.
Notably, researchers documented improvements both one and seven hours after dosing, suggesting cognitive benefits that persisted throughout the day. The confidence intervals for the majority of outcomes supported consistent improvements in attention, memory, and executive function, suggesting dose-related cognitive improvement with oveporexton.
Biological Rationale
The researchers argue that these results support a broader biological theory: orexin (搜索) signaling extends beyond wakefulness to support brain areas responsible for attention, memory, motivation, and decision-making. Supporting evidence from rodent studies shows that damage to orexin-targeted regions hampers learning and working memory, while drugs that block orexin receptors can cause notable cognitive slowing even in healthy adults.
By selectively activating OX2R (搜索), oveporexton may be reactivating a network of cognition-supporting neurons that has been offline since disease onset.
Study Limitations and Future Directions
The study acknowledges several limitations. The trial population was mostly white, and researchers cannot yet determine whether cognitive improvements are a direct effect on neural networks or a secondary effect of better wakefulness.
Despite these caveats, the findings mark a meaningful shift in narcolepsy research.
