Experimental Vaccine Provides Year-Long Protection Against Deadly Allergic Reactions in Mice

The findings, reported by Eva Conde from the Institut Pasteur at Sorbonne Université (搜索) in France and colleagues in Science Translational Medicine, demonstrate the vaccine's potential as a long-term treatment for diseases like asthma (搜索) and food allergies (搜索) that affect as much as a third of the world's population.
Revolutionary Approach to Allergy Prevention
The vaccine targets IgE (搜索) antibodies, which are bound to immune cells in the body's tissues and trigger the release of histamine when exposed to allergens. In severe cases, this can lead to anaphylaxis (搜索), a potentially fatal reaction that causes swelling of the tongue or throat, shock, and difficulty breathing.
IgE-K (搜索) uses kinoid technology, a form of self-immunity training previously used for autoimmune diseases. The researchers took a fragment of human IgE (搜索), engineered it so it couldn't trigger allergic reactions, and fused it to CRM197, a harmless version of diphtheria toxin commonly used in vaccines.
The resulting conjugate vaccine teaches the immune system to recognize and neutralize IgE (搜索). Under the microscope, the IgE fragment is locked in a "closed" conformation that prevents IgE from binding to its receptor, FcεRI (搜索), on mast cells, cutting off the cascade that causes allergic symptoms.
Dramatic Protection in Animal Studies
When tested in "humanized" mice engineered to produce human IgE (搜索) and its receptor, the results were impressive. Two doses of IgE-K (搜索) prompted the animals to make high levels of anti-IgE antibodies that persisted for a year. The vaccine maintained high levels of antibodies against IgE up to 52 weeks after immunization.
The protective effects were dramatic. When challenged with allergens, eight of nine unvaccinated control mice died within 30 minutes of exposure. In contrast, vaccinated mice showed only mild signs of an allergic reaction, and none died. There was no drop in body temperature, no mast cell activation, and no tissue damage.
Even allergy-prone mice bred to overreact to immune triggers were protected. When researchers transferred serum from vaccinated animals into unvaccinated ones, the protection went along with it, proving that antibodies alone were responsible for the protective effect.
Safety Profile and Preserved Immune Function
One major concern was that anti-IgE (搜索) antibodies might accidentally activate mast cells, causing the very reactions they were meant to prevent. While some cross-linking occurred in human cell cultures, this was not observed in living mice. The researchers believe the gradual rise in antibodies after vaccination may "softly desensitize" mast cells, preventing dangerous degranulation.
Importantly, the vaccine didn't compromise the animals' defenses against parasites, which is one of the key roles of IgE (搜索). In infection tests, vaccinated mice fought off nematodes just as well as untreated ones, addressing concerns about maintaining essential immune functions.
Potential Advantages Over Current Treatments
The vaccine works similarly to omalizumab, the first monoclonal antibody approved to treat food allergies (搜索), by blocking IgE (搜索) from binding to immune cell receptors. However, it could offer significant advantages over current treatments.
While omalizumab is effective, it is very expensive and requires repeated, lifelong doses every two to four weeks, making it unsuitable for some patients. The experimental vaccine could provide longer-lasting protection with fewer injections.
"Ultimately, the strong suppression of anaphylaxis (搜索) in the absence of adverse events makes the IgE-K (搜索) vaccine a very enticing therapeutic candidate that fills an important need," wrote Danielle Della Libera and colleagues in a commentary accompanying the paper.
Clinical Implications and Future Outlook
The findings represent an "exciting proof of concept" that a vaccine can trigger the production of antibodies against IgE (搜索), according to Mimi Tang, an allergy and immunology researcher at the Murdoch Children's Research Institute in Melbourne, Australia.
Co-author Laurent Reber, who studies allergic diseases at the Toulouse Institute for Infectious and Inflammatory Diseases (搜索) in France, noted that attempts to create an anti-allergy vaccine in the 1990s were unsuccessful. However, the current research was able to test the vaccine in mice modified to express the human IgE (搜索) receptor, a technology that was not available 30 years ago.
If IgE-K (搜索) or a similar vaccine works in humans, it could mark a paradigm shift in allergy treatment. A single series of injections might replace the chronic cycle of antihistamines, emergency EpiPens, and expensive biologics for managing severe allergic reactions.
"Allergy and anaphylaxis (搜索) are major clinical and financial burdens," Della Libera noted, highlighting the significant unmet medical need that this approach could potentially address.
