The Tumor Monorail (搜索) represents the first-ever bio-inspired implant designed to guide invasive brain tumor cells away from critical brain regions toward a designated "safe zone" outside the brain. The surgically implanted catheter system remains in place throughout treatment, providing continuous access to tumor biology and enabling longitudinal monitoring of tumor evolution and response to therapy.

Combination Immunotherapy Protocol
The Phase 1 study enrolls adults with recurrent glioblastoma (搜索) who have undergone prior standard therapy to evaluate a dual approach combining two investigational immunotherapies with the Tumor Monorail (搜索) monitoring system. The treatment protocol involves D2C7-IT, an investigational immunotoxin designed to target and bind to both wild-type EGFR (搜索) and EGFRvIII (搜索) proteins frequently overexpressed in glioblastoma (搜索) cells, administered in combination with 2141-V11, a fully-human anti-CD40 (搜索) agonist antibody.
The therapeutic regimen begins with insertion of the Tumor Monorail (搜索) in proximity to tumor recurrence, followed by direct intracerebral administration of both D2C7-IT and 2141-V11 using convection-enhanced delivery (CED). This method bypasses the blood-brain barrier to allow high concentrations of drug to reach tumor-infiltrated regions. Patients then undergo repeated dosing of 2141-V11 subcutaneously in the cervical perilymphatic area to maintain immune response, while the Tumor Monorail enables real-time tumor evaluation through subcutaneous needle aspiration at each dosing interval.
Addressing Critical Unmet Medical Need
Glioblastoma (搜索) affects approximately 12,300 Americans annually, with an incidence of 3.2 per 100,000 population. The disease carries a devastating prognosis, with median survival from initial diagnosis of less than 21 months. Current standard of care involves maximal safe surgical resection followed by radiotherapy plus concomitant and maintenance temozolomide chemotherapy, with or without the Optune device. Critically, no standard treatment exists for tumor recurrence.

The study enrolls adults aged 18 or older with histologically confirmed recurrent glioblastoma (搜索) following prior standard therapy including surgery, radiation, and chemotherapy.
