The drug, marketed as Aurlumyn by SERB Pharmaceuticals (搜索), received FDA approval in February 2024 and represents a significant advancement in frostbite (搜索) care. Dr. Deepak Ozhathil, a burn surgeon at Akron Children's, reported treating nearly 20 frostbite patients with iloprost since December, with unprecedented tissue salvage results.
"We've had salvage of digits that, in my clinical experience from the past, I would have expected the patient to lose," Ozhathil said. "By giving Iloprost we've been able to completely change the game in terms of frostbite (搜索) and the resulting amputations."
Extended Treatment Window Transforms Care
The medication addresses a critical limitation in current frostbite (搜索) treatment protocols. Traditional clot-busting drugs must be administered within 24 hours of frostbite injury, but iloprost extends this therapeutic window to 72 hours. The drug can also be used in combination with clot-busting therapy within the initial 24-hour period.
"That's a game changer," Ozhathil explained. "A lot of these patients may not come in within 24 hours, or even two days. Some of them are in denial that they have a problem, or they have limited access to care, so they may not be able to get to a hospital within that time."
Mechanism and Clinical Outcomes
Iloprost works by dilating blood vessels (搜索) and preventing platelet aggregation in frostbitten tissue. When severe frostbite (搜索) occurs, tiny blood vessels become clogged with platelets (搜索) and blood clots, depriving tissue of oxygen and nutrition. The drug restores circulation by opening these vessels and reducing platelet adhesion.
Clinical data demonstrates the treatment's efficacy: in studies measuring amputation rates, zero patients who received iloprost required amputation, while 60% of those who did not receive the medication needed amputation. Success rates have improved from 60-70% to 80-90% range with the new therapy.
Treatment Protocol and Safety Profile
The drug is administered intravenously over five consecutive days, with each infusion lasting approximately six hours. Following the five-day course, patients transition to blood thinners and aspirin therapy. Aurlumyn costs $5,500 per vial without insurance, and multiple vials may be required for a complete treatment course.
The medication demonstrates a favorable safety profile with fewer side effects and lower bleeding risk compared to traditional therapies. According to SERB Pharmaceuticals (搜索), Aurlumyn has no labeled contraindications or known drug interactions. The most common adverse events include headache, flushing, palpitations or tachycardia, nausea, vomiting, dizziness, and low blood pressure.
Limited Adoption Despite European Standard
While iloprost has been standard care in European frostbite (搜索) treatment guidelines, adoption in the United States remains limited. SERB Pharmaceuticals (搜索) reports that more than 30 U.S. institutions are currently using Aurlumyn, with the number of hospitals incorporating the drug into their protocols increasing monthly since the company acquired it in October 2024.

Increased Case Volume Drives Urgency
This winter season has brought an unusually high number of frostbite (搜索) cases to Akron Children's Hospital. "This winter has been really tough, and we have had a lot more patients this year than we saw all of last year, and we're not through the winter yet," Ozhathil noted. As of late January, he had treated four to five frostbite cases within a single week.
Approximately 90% of frostbite (搜索) patients present with housing insecurity, concurrent psychiatric illness, or substance abuse issues. Ozhathil has treated at least two dozen people with housing insecurity during the current season, underscoring the vulnerable population most affected by severe cold exposure.
