FDA Grants Fast Track Designation to 99mTc-Maraciclatide for Interstitial Lung Disease Imaging

The FDA has granted fast track designation to Serac Healthcare (搜索)'s 99mTc-maraciclatide, a SPECT-CT imaging agent designed to visualize inflammation in patients with interstitial lung disease (搜索).
The designation aims to accelerate development of a solution for the critical unmet need of distinguishing between inflammation and fibrosis in ILD (搜索), which comprises over 200 disorders affecting lung function.
Preliminary Phase II data from the PREDICT-ILD (搜索) trial indicate that the radiolabeled tracer may enable feasible visualization of inflammation in patients with fibrotic ILD.
Serac Healthcare (搜索) has received fast track designation from the US Food and Drug Administration for 99mTc-maraciclatide, a SPECT-CT imaging agent intended for the visualization of inflammation in patients with known or suspected interstitial lung disease (搜索). The designation is designed to accelerate the development and review of products that address serious conditions and unmet medical needs.
Addressing Critical Diagnostic Challenges in ILD
Interstitial lung disease (搜索) comprises more than 200 disorders that impair lung function and are marked by progressive inflammation, fibrosis and declining quality of life. Early and accurate differentiation between inflammation and fibrosis is vital for treatment decisions, yet remains extremely challenging with current diagnostic approaches.
"The FDA's Fast Track designation of maraciclatide signals the imperative for improved ILD (搜索) diagnosis, assessment, and monitoring," said David Hail, Chief Executive Officer of Serac Healthcare (搜索). "ILD symptoms are non-specific and often present late in disease progression, making early detection extremely difficult."
The diagnostic challenge extends beyond detection to treatment selection. While symptom management therapies are available, including powerful anti-inflammatory agents, inappropriate administration can prove more detrimental than beneficial, according to Hail.
Mechanism of Action and Clinical Development
99mTc-maraciclatide is a radiolabelled tracer that binds with high affinity to αvβ3 integrin (搜索), a cell-adhesion molecule up-regulated in vascular endothelial cells during angiogenesis, a key process in inflammation. This targeted binding mechanism enables the agent to potentially distinguish between inflammatory and fibrotic processes in lung tissue.
Preliminary phase 2 data from the PRospective Evaluation of Interstitial Lung Disease (搜索) progression with quantitative CT (PREDICT-ILD (搜索)) trial indicate that visualization of inflammation in people with fibrotic ILD may be feasible using the agent. Further clinical results are expected later this year.
Fast Track Benefits and Development Timeline
The fast track designation provides several potential advantages intended to shorten the time to approval in the US, including eligibility for accelerated approval and priority review, more frequent FDA meetings and written communication, and rolling review of sections of a new drug application as they are completed.
"A non-invasive imaging solution capable of distinguishing inflammation and fibrosis predominant ILD (搜索) has the potential to meaningfully advance early diagnosis, change the treatment paradigm and improve patient outcomes," Hail noted.
The agent remains investigational and is not approved by the FDA or UK and European regulatory authorities.
