enVVeno Medical Corporation (搜索) received an unfavorable decision from the U.S. Food & Drug Administration on November 13, 2025, upholding the agency's rejection of the VenoValve (搜索) surgical replacement venous valve for treating severe deep chronic venous insufficiency (搜索) (CVI). The supervisory appeal upheld the review staff decision that the VenoValve did not meet the standard of reasonable assurance of safety and effectiveness.
The FDA's initial not-approvable letter, issued on August 19, 2025, cited concerns about the device's benefit-risk profile despite favorable clinical data. The agency determined that positive results from the revised Venous Clinical Severity Score (rVCSS), along with improvements in pain scores and venous-specific quality of life indicators, were insufficient to establish the device's favorability. The FDA raised concerns about potential bias and questioned whether clinical improvements resulted from study participation rather than the device itself, noting the absence of specific hemodynamic measurements correlating with patient improvement.
Safety Concerns Drive Regulatory Decision
Safety issues attributed to the VenoValve (搜索)'s open surgical procedure, including required re-hospitalizations, factored significantly in the FDA's decision. The agency's concerns centered on complications associated with the surgical approach required for device implantation.
"We are obviously disappointed by the FDA's decision. The results showed that a high percentage of the patients in the SAVVE study, who all previously failed standard of care treatments, showed significant clinical improvement after receiving the VenoValve (搜索)," said Robert Berman, enVVeno Medical's Chief Executive Officer.
Strategic Pivot to Next-Generation Technology
Following the appeal decision, enVVeno announced plans to redirect resources toward enVVe (搜索), its transcatheter-based replacement venous valve currently in development. The company believes this non-surgical approach may address the safety concerns that led to VenoValve (搜索)'s rejection.
"enVVe (搜索) should have a different safety profile than an open surgical device and is ready for human testing," Berman stated. "Assuming that we can reach alignment with the Agency on achievable endpoints for enVVe, it makes sense to turn our attention and devote our resources to enVVe."
Significant Unmet Medical Need
The regulatory setback impacts treatment options for a substantial patient population. Severe deep venous CVI affects approximately 2.5 million to 3.5 million patients in the U.S., including approximately 1.5 million patients who develop venous leg ulcers (搜索). The condition, most often caused by blood clots in the deep veins of the leg, severely impacts everyday functions including sleeping, bathing, dressing, and walking, and results in high rates of depression and anxiety.
Currently, no effective treatments exist for severe CVI of the deep vein system caused by valvular incompetence. The economic burden is substantial, with the average patient seeking venous ulcer treatment spending as much as $30,000 annually on wound care. Total direct medical costs from venous ulcer sufferers in the U.S. are estimated to exceed $20 billion annually.
Financial Position and Future Operations
Despite the regulatory setback, enVVeno maintains a strong financial position with $31.5 million in cash and investments at the end of the third quarter. With quarterly cash burn of $4 to $5 million, the company has sufficient funding to continue operations into 2027.
The company plans to continue interactions with the FDA regarding enVVe (搜索) development and will provide periodic updates on progress. enVVeno is currently performing final testing necessary to seek approval for the pivotal trial for enVVe, applying key learnings from the VenoValve (搜索) approval process to its next-generation device development.
