Federally funded cancer (搜索) clinical trials play an indispensable role in addressing critical gaps left by pharmaceutical industry research, particularly in areas involving rare cancers (搜索), pediatric populations, and complex treatment combinations, according to new research presented at the 2025 American Society of Clinical Oncology (ASCO (搜索)) Quality Care Symposium.
Joseph Unger, PhD, of Fred Hutchinson Cancer Center, and colleagues analyzed data from the National Institutes of Health's ClinicalTrials.gov registry spanning 2008 to 2024, examining U.S.-based trials that included drugs or biological agents for cancer (搜索) treatment. The comprehensive analysis, funded by the Hope Foundation for Cancer Research (搜索), revealed stark differences in research priorities between federal and industry funding sources.
Funding Distribution and Focus Areas
Of the 10,142 cancer (搜索) clinical trials included in the analysis, only 1,711 (16.9%) received federal funding, while 8,431 (83.1%) were industry sponsored. Despite representing less than one-fifth of all trials, government-funded studies demonstrated a markedly different research focus compared to their industry counterparts.
Government-funded trials showed significantly higher involvement in rare cancers (搜索) (17.8% versus 12.3%) and pediatric cancers (搜索) (16.2% versus 5.1%). This disparity highlights the federal government's role in supporting research areas that may not offer sufficient commercial incentives for pharmaceutical companies.
"Federally funded cancer (搜索) clinical trials play an important role in evaluating interventions and improving cancer outcomes in settings and populations that are not prioritized by industry," said ASCO (搜索) chief medical officer and executive vice president Julie Gralow, MD. "With more than 18 million cancer survivors in the United States, federal research has delivered an undeniable return on investment."
Trial Phase and Treatment Complexity
The analysis revealed distinct patterns in trial phases between funding sources. Federally funded studies were more likely to be early and mid-stage trials, with 87.3% being Phase I or II studies that assess pharmacokinetics, optimal dose, safety, tolerability, and preliminary efficacy, compared to 82.0% for industry trials.
Conversely, industry studies more frequently conducted Phase III trials (18.0% versus 12.7%), which test new therapies in larger populations to collect safety and effectiveness data supporting regulatory approval.
Perhaps most notably, federally funded studies demonstrated greater complexity in treatment approaches. While industry trials focused predominantly on single-agent drug studies (79.9% versus 50.8% for federal trials), government-funded research more often involved complex multimodality combinations.
Federal trials showed substantially higher rates of drug or biological agent combinations (20.2% versus 7.7%), combined drugs or biologicals with radiation (11.0% versus 1.5%), surgery (2.6% versus 0.2%), transplantation (1.5% versus 0.1%), or mixed modalities (1.9% versus 0.1%).
Dose De-escalation Research
The federal government also demonstrated significantly greater investment in dose de-escalation trials, funding such studies at a rate of 3.2% compared to industry's 0.4%. These trials, introduced in the 1970s and increasingly relevant in the precision medicine era, aim to safely reduce the intensity, duration, or dose of cancer (搜索) therapies to minimize toxicities, side effects, and costs while maintaining therapeutic effectiveness.
Geographic Access and Rural Healthcare
A separate study presented at the meeting examined the geographic distribution of federally funded cancer (搜索) clinical trials, revealing the critical role of government funding in ensuring access across diverse communities.
"We found that more than a quarter of the 903 counties with trials had only federally sponsored trials and most rural and high cancer (搜索) mortality counties with trials had only federally-sponsored trials," said study coauthor Ishwaria Subbiah, MD, of the Sarah Cannon Research Institute (搜索).
The research built upon a 2024 ASCO (搜索) analysis showing that 70% of U.S. counties lacked cancer (搜索) trials entirely. Counties with sites supported by National Cancer Institute infrastructure awards were far more likely to have trials funded by any sponsors, underscoring the foundational role of federal investment.
Future Research and Sustainability
The research team plans to examine how the increasing dominance of industry-sponsored cancer (搜索) trials over recent decades is shaping patient access to studies. This future research aims to identify gaps and ensure both federally funded and industry-funded studies better serve patients.
"Federal trials fill critical gaps left by industry, especially in early-phase research, multimodality treatments, rare cancers (搜索) and treatments for children," Unger emphasized. "The trials that address these types of research questions are vital for patients and their families, even if the outcomes of such trials are not profitable for private companies."
The findings underscore the necessity of sustained federal investment in cancer (搜索) research. "Sustained or increased funding is essential to maintain progress in these areas and to maintain a healthy cancer research ecosystem that balances federally sponsored and industry-sponsored trials," Unger concluded.
The research illustrates that federal funding underpins the robust network of cancer (搜索) clinical trials in the United States, with many patients—particularly those in rural areas and regions with high cancer mortality—depending on federal trial sponsorship for the opportunity to participate in research near their homes.
