Roctavian represents the first FDA-approved gene therapy for adults with severe hemophilia A (搜索) who do not have antibodies to the AAV5 (搜索) virus. "The gene goes into the patient's liver and it becomes a part of the patient's liver and starts living there and making more protein," Kuriakose explained. "This protein that is made, the factor VIII (搜索) in this instance, is made at a steady level. So there are none of these ups and downs. No peaks and valleys."
The therapy aims to transform patients with severe hemophilia requiring frequent infusions into those with mild hemophilia, only needing treatment during surgery or after injuries. Kuriakose anticipates that after six months, as levels stabilize and side effects remain minimal, patients "can get back into much of life, like most of us do, without thinking twice."

However, Kuriakose suggests that over a lifetime, gene therapy might prove more cost-effective than continuous traditional treatments.
Rox Gatia, vice president of pharmacy shared services at Henry Ford Health (搜索), noted that insurance coverage remains restrictive. "Most insurance companies have very strict guidelines on gene therapies, especially for cellular and gene editing, and who can access them, so not everyone can get them," Gatia explained. The recent Michigan patient's treatment was covered by insurance.
Expanding Gene Therapy Program
Henry Ford Health (搜索) has developed comprehensive infrastructure to support gene therapy delivery over the past four years, including dedicated cell and gene therapy pharmacy specialists. The program extends beyond hematology to oncology, rare genetic diseases, and other emerging therapies.

The recent Michigan patient's infusion took over two hours to complete, though duration varies among patients. Post-treatment monitoring includes regular blood draws to ensure proper response and monitor for side effects, particularly liver-related complications.
Kuriakose emphasized that while gene therapy doesn't cure hemophilia, it provides effective long-term control. "You're not curing it with gene therapy, you're controlling it in that patient, and allow for a lifetime of, hopefully, healthy living without toxicity."
