The randomized, placebo-controlled trial, published in the Annals of Internal Medicine, found that fluvoxamine significantly improved fatigue (搜索) and quality of life among adults with long COVID (搜索). The study represents a breakthrough for a condition that has left many patients unable to work or resume normal activities.
Trial Design and Methodology
The REVIVE-TOGETHER trial was co-led by researchers from McMaster University, the University of British Columbia, Stanford University, and multiple Brazilian institutions. The study enrolled 399 adults in Brazil who continued to experience fatigue (搜索) for at least 90 days after a confirmed SARS-CoV-2 infection.
Participants were randomly assigned to receive fluvoxamine (marketed as Faverin or Luvox), metformin, or a placebo for 60 days. The researchers used a sophisticated Bayesian adaptive design, which allowed them to stop treatment arms early when results became clear.
"We wanted to test whether two existing, widely available, and affordable medications could help. Both had biological reasons to think they might work against long COVID (搜索) fatigue (搜索), but neither had been rigorously tested for this purpose in a proper clinical trial," said Edward Mills, senior author and professor in McMaster's Department of Health Research Methods, Evidence, and Impact.
Key Findings
The trial demonstrated that fluvoxamine reduced fatigue (搜索) more than placebo, with evidence suggesting a 99% probability that the drug outperformed placebo. Patients taking fluvoxamine reported steady improvements in fatigue and quality-of-life scores over 60 to 90 days, with fewer side effects than placebo.
In contrast, metformin showed no significant benefit for established long COVID (搜索) fatigue (搜索) symptoms, despite previous research showing it can reduce the risk of developing long COVID when taken during acute infection.
"The fluvoxamine patients reported steady improvements in fatigue (搜索) and quality-of-life scores over 60 to 90 days, with fewer side effects than a placebo," according to the study findings.
Clinical Significance
Long COVID (搜索) affects an estimated 65 million people worldwide, with fatigue (搜索) being the most common and debilitating symptom. Currently, most medical guidelines offer only supportive care, such as pacing and symptom management, because proven treatments have been lacking.
"This is an important step forward for patients who have been desperate for evidence-based options," Mills noted. "Fluvoxamine showed consistent and meaningful benefits, and because it's already widely used and well understood, it has clear potential for clinical use."
Mechanism of Action
Fluvoxamine belongs to a class of drugs called selective serotonin reuptake inhibitors (SSRIs), which boost serotonin in the brain to improve mood. However, the drug also appears to calm the immune system (搜索), preventing it from overreacting, which may explain why it eases long COVID (搜索) fatigue (搜索).
Previous studies have suggested that long COVID (搜索) may be caused by the immune system (搜索) remaining overactive even after the initial infection has been cleared, making fluvoxamine's immune-modulating properties particularly relevant.

The researchers acknowledge important limitations in their study. The trial did not assess participants' history of depression, making it unclear whether the effect was a direct impact on long COVID (搜索) or related to underlying depression. Additionally, the study excluded patients with depressive or anxiety disorders and focused primarily on fatigue (搜索) without assessing other long COVID features such as post-exertional malaise, autonomic symptoms, or cognitive impairment.
Professor Christiaan Vinkers from Amsterdam University Medical Centre, who was not involved in the study, cautioned that while the findings are promising, "the pill is unlikely to be a golden bullet." He emphasized that "replication is essential, ideally in broader patient groups and with outcomes that capture the full spectrum of long Covid (搜索)."
Clinical Implementation
The study's corresponding author, Jamie Forrest, a postdoctoral research fellow at the University of British Columbia, highlighted the practical implications: "This trial gives clinicians their first strong evidence for a medication that helps reduce long COVID (搜索) fatigue (搜索). Patients want something they can try today – and this finding brings us closer to that reality."
The researchers emphasize that while fluvoxamine offers a promising option for managing fatigue (搜索), long COVID (搜索) is a complex condition with multiple symptoms and biological pathways. Further studies are needed to understand who benefits most, how the medication works, and how it might be combined with other emerging treatments.
