The study, published in Blood, tested the product on blood vessel cells in laboratory dishes, 3D organoid models, and mice with brain injuries. Mice receiving Thrombosomes either one hour or one day after injury showed significantly less hemorrhage (搜索), reduced blood vessel leakage, and decreased brain inflammation that leads to life-threatening swelling.
Addressing Critical Treatment Gap
Traumatic brain injury (搜索) represents the leading cause of death in people under 44 years old, yet treatment options remain severely limited. "In some cases, surgeons will remove part of the skull to relieve the pressure, but there's no drug that effectively treats swelling, or cerebral oedema, directly," said Dr. Shibani Pati, director of the UCSF Center for Research Transfusion Medicine and Cell Therapies and senior author of the study.

Fresh platelets are currently used to treat hemorrhage (搜索) in cancer patients and prevent bleeding during surgery, but they have not proven effective against TBI (搜索). The global shortage of fresh platelets has driven research into preserved alternatives for the past 30 years, though no preserved platelet product has received approval for human use.
Mechanism of Action
The research team discovered that Thrombosomes contain high concentrations of Angiopoietin-1 (搜索), a protein that activates receptors on blood vessel cells to stabilize them and reduce leakage. This mechanism may explain the product's effectiveness in reinforcing damaged brain blood vessels.
"Platelets carry many potent factors that go beyond clotting," Pati explained. "In our mouse model of TBI (搜索), we saw hints that this product concentrates these factors, making it more effective than platelets themselves."
Transcriptomic analysis of brain tissue revealed that Thrombosomes downregulate gene networks associated with inflammation and fibrosis, suggesting the product modulates post-injury repair processes. The treatment also suppressed intravascular leukocytosis and reduced microglial activation, astrocyte reactivity, and macrophage infiltration.
Clinical Development Pathway
The product's development timeline could be accelerated because Thrombosomes are already undergoing Phase II clinical trials for bleeding disorders, establishing their safety profile in humans. This existing safety data could significantly hasten trials testing the product specifically for TBI (搜索) applications.
Originally developed for battlefield hemorrhage (搜索) control, Thrombosomes represent what researchers describe as a "first-in-class" freeze-dried platelet-derived biologic. The product's ability to be stored on ambulances or in remote emergency departments could transform acute TBI (搜索) care, particularly in settings where fresh platelets are unavailable.
The research was supported by the Department of Defense, reflecting the military's interest in developing portable treatments for combat-related brain injuries. The findings suggest that Thrombosomes could fill a critical gap in TBI (搜索) therapeutics by targeting both immediate bleeding and the delayed vascular instability that contributes to cerebral edema (搜索).
