When Heidi Griffin arrived at St. Jude Children's Research Hospital in 1967 as its 860th patient, the outlook for children diagnosed with acute lymphoblastic leukemia (搜索) (ALL) was grim. Survival rates hovered around 4%, and the hospital had opened its doors just five years earlier. Today, thanks to decades of iterative clinical research, that survival rate stands at 94% — a transformation driven by St. Jude's pioneering Total Therapy protocols.
Griffin, diagnosed at age 2, was enrolled in the Total Therapy 5 clinical trial alongside 34 other children with ALL. The trial marked a significant departure from previous protocols: while earlier St. Jude approaches had relied on radiotherapy to both the head and spine to prevent disease relapse, Total 5 employed an increased radiotherapy dose targeted only to the head and used chemotherapy instead of radiation to treat the spine.
The results were striking. Roughly half of the 35 children in the study were cured and survived long-term. Griffin was among them. "It was very amazing, that trial," she reflected. "I was very lucky to be alive because of the testing that was done."
Building on Decades of Data
The Total Therapy approach, developed and refined at St. Jude over decades, raised survival rates for childhood cancer from below 20% to above 80%. Scientists at the institution systematically collected genetic information and clinical outcomes data to understand which patients would respond to specific treatment regimens. This cumulative knowledge now informs contemporary trials.
The INITIALL clinical trial, led by Seth Karol, MD, MSCI, of the Department of Oncology, represents the modern evolution of this research philosophy. INITIALL studies a new approach to pediatric leukemia care, designed to help doctors match children with the treatments most likely to benefit them. The trial uses the institution's extensive repository of genetic and outcomes data to guide clinicians in determining what treatments children with leukemia should receive.
A Family's Journey Through Modern Trials
Eleven-year-old Weston, diagnosed with T-cell acute lymphoblastic leukemia (搜索) (T-ALL), was enrolled in INITIALL after arriving at St. Jude in critical condition. The guidance from INITIALL led to the addition of an extra round of chemotherapy as part of his treatment on the SJALL23T trial, which uses INITIALL data to modify therapy for children with T-ALL.
"Thankfully, it seems to be working, and Weston is in remission now," said his mother, Joni. "But he has to continue the treatment for two and a half years, which is hard. He can't go back to school with his friends yet, and he's restricted from a lot of other activities, but the people at St. Jude are helping us emotionally."
The emotional support proved critical from the outset. "One of the first things Dr. Karol told me was 'this was not your fault,' which was a thought that had haunted me since learning Weston's diagnosis," Joni said. "He's very comforting, but he's also honest with you about the situation."
The Survivorship Perspective
More than five decades after her initial treatment, Griffin has returned to St. Jude as a participant in two research studies examining the long-term effects of treatment and health outcomes for survivors of childhood cancer. The information gathered from these studies helps inform future treatments for patients and care plans for survivors.
Now in her 60s, Griffin has included St. Jude in her estate plans, naming the hospital as a beneficiary of both her will and trust. "It is important to me to give back," she said. "St. Jude saved my life, and I owe my life to St. Jude."
The arc from Total Therapy 5 to INITIALL and SJALL23T illustrates how clinical research, built on incremental advances and close collaboration between families and care teams, has fundamentally altered the prognosis for childhood leukemia. As Griffin observed while walking the expanded campus decades after her treatment: "It means they're doing a good job."
