The study, led by Franco Locatelli and colleagues, enrolled 54 children with high-risk metastatic, relapsed, or treatment-refractory neuroblastoma across both the clinical trial and case series.
Clinical Efficacy Results
The trial demonstrated an overall response rate (ORR) of 66% in the clinical trial cohort, with a five-year overall survival of 42.7% (excluding three patients treated with no evidence of disease). Children with low disease burden who received the recommended CAR T dose showed even more promising outcomes, achieving an ORR of 77% and a five-year overall survival of 67.6%. This subgroup included 30 patients across both the trial and case series.
CAR T Cell Persistence and Safety
The GD2-targeting CAR T cells, designated as GD2-CART01, exhibited robust expansion and persistence following infusion. Remarkably, the cells remained detectable up to five years post-infusion in some patients with sufficient follow-up. The study reported no new safety signals beyond those previously identified in the interim analysis published in the New England Journal of Medicine in 2023.
Significance for Solid Tumor Treatment
These results represent a significant advancement for CAR T cell therapy in solid tumors, an area where clinical efficacy has been limited compared to hematological malignancies. Neuroblastoma (搜索), one of the most common pediatric cancers, expresses high levels of GD2 (搜索), making it an attractive target for this therapeutic approach. Future Development
Based on these encouraging results, a larger phase 2 trial is now being planned to validate this therapeutic approach. The study included 35 patients in the formal clinical trial and an additional 19 children treated under hospital exemption with the same criteria, providing a robust dataset for evaluating the therapy's potential.
