The patient, Karnan Madhusudhanan, initially presented with persistent headache, giddiness, difficulty walking, and weakness in the left arm and leg. Detailed investigations confirmed the diagnosis of PCNSL, a rare brain lymphoma that poses substantial therapeutic challenges.
Disease Progression Despite Standard Therapies
The patient's clinical course was marked by refractory disease. He initially received high-dose chemotherapy, the standard first-line treatment for PCNSL. However, the lymphoma continued to progress after four cycles. The clinical team subsequently administered radiotherapy combined with chemotherapy and oral targeted therapy as a bridging strategy. Despite these interventions, the disease remained resistant to treatment.

These cells were then genetically modified to recognize and attack lymphoma cells, and subsequently reinfused into the patient as CAR-T cells. The engineered cells are designed to identify, engage, and eliminate cancer cells expressing the CD19 antigen.
Complication Management and Recovery
Following the CAR-T cell infusion, the patient developed several serious health complications associated with CAR-T cell therapy. A dedicated multidisciplinary team comprising specialists from medical oncology, hematology, infectious diseases, intensive care, and nursing provided comprehensive supportive care. After approximately one month of intensive treatment, the patient recovered sufficiently and was discharged.
