Two comprehensive studies published in leading dermatology journals provide compelling evidence for incorporating these medications into treatment protocols for this challenging chronic inflammatory skin condition.
Clinical Efficacy Across Multiple Studies
A systematic review published in the Journal of the American Academy of Dermatology analyzed six studies encompassing 92 participants with concurrent hidradenitis suppurativa (搜索) and obesity (搜索) or diabetes (搜索). The results showed that 58.6% of participants experienced improvement in their hidradenitis suppurativa after initiating GLP-1 therapy. Quality of life measures, as assessed by Dermatology Life Quality Index (DLQI) scores, demonstrated substantial improvement, falling from 13.05 before treatment to 9.19 after treatment.
The majority of patients (81.5%) received semaglutide, while 17.4% were treated with liraglutide and 1% received tirzepatide, with varying dosages and treatment durations across the studies.
French Multi-Center Analysis Confirms Benefits
A larger retrospective analysis published in JAMA Dermatology tracked 66 adult patients across 15 centers in France from 2017 to 2024. With a median follow-up of 18.5 months, the study provided more robust long-term data on GLP-1 efficacy in hidradenitis suppurativa (搜索) management.
The patient population had a median age of 46.0 years and median body mass index of 39.4, with 86% having concurrent diabetes (搜索). All participants had Hidradenitis Suppurativa (搜索) Physician's Global Assessment (HS-PGA) scores of 1 or higher, indicating active disease.
At the 6-month mark, 54% of patients achieved at least a 1-point reduction in HS-PGA scores, with 12% experiencing a 2-point or greater reduction. These improvements continued to strengthen over time, with final follow-up results showing 62% of patients achieving at least a 1-point HS-PGA reduction.
Comprehensive Symptom Improvement
Beyond disease severity scores, patients experienced broad-based improvements across multiple clinical parameters. Pain reduction was reported by 60% of patients, while 67% saw decreased frequency of hidradenitis suppurativa (搜索) flares. Importantly, these benefits were observed even in a subgroup of patients whose other hidradenitis suppurativa treatments remained unchanged, suggesting a direct therapeutic effect of GLP-1 receptor (搜索) agonists.

Clinical Implementation Considerations
Dr. John Barbieri, assistant professor at Harvard Medical School and director of the advanced acne therapeutics clinic at Brigham and Women's Hospital, emphasized the clinical significance of these findings. "This is particularly important as we need more effective treatments for moderate-to-severe disease. GLP-1 receptor (搜索) agonists may offer a new approach that can complement existing topical, systemic and biologic treatment options," Barbieri told Healio.
The research suggests particular benefit for patients with recalcitrant disease and those for whom weight loss could improve disease severity and quality of life. More than half (53%) of patients in the French study were receiving concurrent hidradenitis suppurativa (搜索) treatments, including antibiotic therapy or biologics, indicating the potential for combination therapeutic approaches.
Future Research Directions
While the current evidence is encouraging, researchers acknowledge limitations in the existing data. "Although the identified studies were typically small and mostly observational, they highlight the potential role of GLP-1 receptor (搜索) agonists in the management of HS," Barbieri noted.
The retrospective design of the larger French study and concurrent use of other hidradenitis suppurativa (搜索) medications in some patients prevent definitive conclusions about GLP-1 monotherapy efficacy. "Randomized clinical trials are warranted to confirm the role of GLP-1 RAs in hidradenitis suppurativa management," the JAMA Dermatology authors concluded.
Expanding Treatment Paradigms
The research suggests potential benefits extending beyond patients with obesity (搜索). "GLP-1 RAs offer promise for patients with hidradenitis suppurativa (搜索) and obesity and potentially for patients without obesity through immunological effects," researchers noted, indicating broader therapeutic applications.
