Landmark Clinical Trial Results
The multinational phase 3 trial, led by Dr. Ioannis Mountzios of Henry Dunant Hospital (搜索) in Athens, compared the immunotherapy drug tarlatamab with standard chemotherapy in 509 patients whose small cell lung cancer had returned or progressed after platinum-based chemotherapy. The study randomly assigned patients to receive either tarlatamab or chemotherapy drugs, including topotecan, lurbinectedin, or amrubicin.
Results from the interim analysis showed patients treated with tarlatamab lived significantly longer, with median overall survival reaching 13.6 months compared with 8.3 months for those receiving chemotherapy. This represents an improvement of more than five months in survival time.
Mechanism and Administration
Tarlatamab is described as a bispecific delta-like ligand 3 (搜索)-directed T-cell engager that works by activating the body's immune cells to target and attack cancer cells. As Dr. Mountzios explained, "This innovative treatment essentially brings the cancer cell and the patient's immune cell together, enabling the immune system to recognize and attack the cancer."
The new immunotherapy is administered intravenously, with an initial induction phase of weekly infusions for three weeks (days 1, 8, and 15), followed by infusions every two weeks for as long as the patient benefits.
Improved Safety Profile and Quality of Life
Beyond survival benefits, the study demonstrated significant improvements in patient safety and quality of life. Patients receiving tarlatamab experienced fewer severe side effects—54% compared with 80% in the chemotherapy group—and were less likely to discontinue treatment due to adverse events.
"It is not chemotherapy with all that this entails," Mountzios clarified. "Therefore, it does not cause hair loss, it does not lower blood counts (hematocrit, white blood cells, platelets), and it lacks most of the classic side effects we associate with chemotherapy."
The treatment also provided clinically significant improvements in bothersome symptoms like shortness of breath and cough, leading to a noticeably better quality of life for patients.
Addressing an Urgent Medical Need
Small cell lung cancer accounts for approximately 15-20% of lung cancer cases worldwide and is strongly linked to smoking. The disease exhibits the most aggressive biological behavior among lung cancer types, with an average survival time of only twelve to thirteen months from diagnosis.
For thirty years, chemotherapy was the primary treatment for SCLC, with immunotherapy being a very recent addition. Treatment options for patients who relapsed after initial chemotherapy and immunotherapy were extremely limited, offering an average survival of only around eight months post-relapse.
Global Regulatory Approval
The therapy has already secured accelerated approval in several countries, including the United States, Canada, Brazil, the United Kingdom, Japan, and Australia. Broader European approval, including in Greece, is anticipated by 2026.
Greek Research Excellence
The study underscores Greece's growing capabilities in international clinical research. Henry Dunant Hospital (搜索) enrolled more patients in the trial than any other institution worldwide, surpassing renowned centers such as Memorial Sloan Kettering Cancer Center in New York and Shanghai University Hospital (搜索).
"Small countries like our homeland can contribute substantially to these global research efforts," Mountzios emphasized. He called the achievement "a defining moment for Greek oncology" and evidence that local research centers now stand alongside the best in the world.
The research involved over eighty centers worldwide and represents a significant milestone in cancer treatment, offering hope for patients facing one of the most challenging forms of lung cancer.
