Gut Health Experts Cut Through the Hype: Fiber, Fermented Foods and Food-Safety Caution Outrank Probiotic Pills

Leading gut researchers caution that there is no clinical consensus on what defines a "healthy" microbiome, making direct-to-consumer microbiome tests clinically uninformative.
Experts emphasize that dietary fiber and fermented foods are the most powerful, evidence-backed ways to support the gut microbiome, rather than expensive probiotic supplements.
A gastrointestinal physiologist has identified the five foods he personally avoids, underscoring an often-overlooked dimension of gut health: food safety. Jordan Haworth, a gastrointestinal physiologist at the Functional Gut Clinic (搜索) in the U.K., has spent nearly a decade helping patients address the root causes of their digestive issues. "When I first started, I didn't realize gut health was a thing," Haworth said. "Now we know just how important gut health is to our overall health. It influences our mood, metabolism, skin, and immunity."
Haworth's guidance arrives amid surging public interest in the gut microbiome, the vast community of trillions of bacteria and other microbes living in the digestive system that help digest food, produce beneficial compounds, support the immune system, and protect against harmful pathogens. Yet awareness remains uneven. A 2024 survey commissioned by Danone North America found that 41 percent of Americans had never heard of the gut microbiome, while 50 percent did not realize it can affect gut health.
The Foundations of Gut Health
Haworth argues that gut health is frequently overcomplicated and has been co-opted by commercial interests. "The term has been hijacked by supermarkets and supplement companies," he said. "The good news is, gut health doesn't need an expensive probiotic pill or green powder."
Instead, he recommends focusing on what he calls the "three Fs": fiber (beans, oats, apples, pears, whole grains, chia seeds, and psyllium); phenols, or polyphenols (compounds found in colorful and bitter foods such as blueberries, red cabbage, coffee, dark chocolate, and olive oil); and fermented foods (yogurt, kefir, kimchi, and sauerkraut). He also advises prioritizing sleep, managing stress, and getting at least 150 minutes of exercise each week.
Why Food Poisoning Matters
According to Haworth, avoiding food poisoning is an often-overlooked pillar of protecting gut health. "Food poisoning is the single biggest wrecker of gut health," he said. "This is because the bugs that cause foodborne illness can have long-lasting effects on the gut."
He points to research suggesting that around one in five people develop post-infectious irritable bowel syndrome (搜索) (IBS) following a Campylobacter (搜索) infection. "This is called post-infectious IBS," Haworth said. "We think it's because the different bacteria that cause food poisoning all share one thing in common: they produce a toxin called CdtB (搜索). Research shows this toxin can damage the gut, leading to long-term inflammation and increased sensitivity, meaning you may become intolerant to foods you previously handled just fine."
While proper food storage and cooking are important, Haworth acknowledges that many cases of food poisoning occur outside the home. "For that reason, I tend to avoid common culprits where possible, like buffets and kebabs," he said.
The Five Foods to Avoid
Haworth's list begins with raw oysters. "Oysters are filter feeders," he said. "That means they pick up everything that's in the surrounding water, including bacteria and other pathogens." He would only eat them again if they had been cooked or undergone depuration, a process using UV-treated water to reduce potentially harmful bacteria.
Second are rare beef burgers. Unlike steak, burgers are made from minced beef, meaning any bacteria on the surface of the meat can become distributed throughout the patty during processing. "I think it mostly happens at restaurants where they ask how you like the burger cooked," Haworth said. "We instinctively think it's a steak, so we're inclined to choose it on the rarer side."
Third is sushi. "Sushi is usually made with sushi-grade fish," he said. "This is where the fish is deep-frozen to kill parasites. The problem is, this doesn't necessarily eliminate all bacterial risks. Once sushi is prepared, it needs to be eaten quickly." He advises caution with heavily discounted sushi that has been sitting on shelves for extended periods.
Fourth are sprouts, which carry an elevated food-safety risk despite their health-food reputation. "Firstly, how they're grown: warm and wet, which are perfect conditions for bacterial growth," he said. "They're also grown close to the soil, where bacteria live. The second reason is that they're usually eaten raw."
Finally, Haworth flags pre-packaged salad bags, which carry increased risk because of the numerous opportunities for contamination during harvesting, processing, packaging, and transportation. The CDC has identified leafy greens and other raw produce as foods frequently implicated in food poisoning outbreaks because they are often eaten without cooking. "You only have to look at the recent Cyclospora outbreak in the U.S., which was linked to iceberg lettuce," Haworth said. "Those mixed salad bags are something to be cautious of because you have different leafy greens coming from different sources, meaning more people handling them and more opportunities for cross-contamination." His advice is direct: "Stay away from these pre-prepared bags. If I want to eat spinach, I'll buy spinach by itself and wash it thoroughly."
What a "Healthy" Microbiome Actually Means
Haworth's practical guidance aligns with a broader caution from leading gut researchers about the limits of current science. Hannah Holscher, a professor of nutrition at the University of Illinois in Urbana-Champaign, notes that despite major strides in the field, there is "no clinical or scientific consensus around what constitutes a healthy microbiome." Because the specific makeup of the gut microbiome varies considerably based on geography, lifestyle, diet, genetics and other factors, there is no single metric that can deliver a straightforward assessment of gut health in the way cholesterol or blood pressure can.
Scientists do, however, have clues about desirable characteristics. Research shows a higher diversity of microorganisms can be one indicator, along with how well the microbiome resists and recovers from disruptions such as illness or antibiotics. A major study published last year surveyed more than 40,000 human gut microbiomes from dozens of countries and ranked groups of bacteria that appear to be "keystones" of health and resilience, including Roseburia, Eubacterium, Faecalibacterium, Allistipes and Bacteroides. Holscher notes these top-ranking microbes share common themes, including their ability to ferment fiber, produce healthful metabolites and make short-chain fatty acids. For example, Roseburia is a major producer of butyrate, a short-chain fatty acid that nourishes colon cells, supports the gut lining and helps create an acidic environment inhospitable to pathogens like E. coli.
Dr. Alexander Khoruts, a professor at the University of Minnesota who directs the UMN Microbiota Therapeutics Program, cautions against simplistic notions of "good" and "bad" microbes. "There's a new understanding that we're not necessarily talking about just one microbe at a time causing problems," he said. "It could be an entire microbial community as a unit that may be dysfunctional."
Questioning Microbiome Tests and Probiotics
The absence of a clinical definition of a healthy microbiome has direct implications for the booming direct-to-consumer testing market. These tests typically rely on stool samples, cost a few hundred dollars and deliver hundreds of measurements related to microbes and metabolites, sometimes offering a gut health score. Khoruts says patients frequently present him with such results. "I think the message the patient gets is 'gee, my doctor never did this kind of testing — it must mean something,'" he said. "The reality is, unfortunately, it's not very informative at all." Because there is no agreed-upon metric for a healthy microbiome and so much variation exists among individuals, these tests are essentially "meaningless" from a clinical standpoint. Research also shows technical issues: the same stool sample sent to different companies can deliver different results depending on the methods used.
Probiotics (搜索) face a similar scrutiny. Experts outline three core requirements: the microbes must be alive, present in sufficient quantities and have a proven health benefit. Dr. Fergus Shanahan, an emeritus professor of medicine at the National University of Ireland in Cork, says the last criterion is usually the one commercial products gloss over by claiming they have been "clinically tested" while omitting underwhelming findings. "I'm afraid the consumer has to be really careful about that," he said.
Researchers do agree that probiotics (搜索) have potential, and some strains are backed by high-quality data showing effectiveness in specific circumstances — such as lowering the risk of diarrhea from antibiotics and severe complications for preterm infants, as well as positive data on prevention of respiratory tract infections and travelers diarrhea, and in the management of some inflammatory bowel diseases. Maria Marco, a professor of food technology at UC Davis, clarifies a common misperception: probiotics are not intended to colonize the gut permanently, but rather to temporarily trigger "our immune system, our intestinal lining and our gut brain-axis" to make beneficial compounds while being taken. "The challenge probiotics face is the same issue with gut microbiome — that it depends on which microbe, for which person, at which time," she said.
Fiber and Fermented Foods: The Evidence-Backed Path
Experts consistently point to fiber as the single most powerful and simplest way to bolster the gut microbiome. Most Americans fall short of the recommended daily goal, which ranges from approximately 20 grams to about 40 grams depending on age and sex. Holscher emphasizes the value of diversity: "Like an apple tastes different from an orange tastes different from an avocado, they have different fibers in them and different phytonutrients. Different microbes can go in and use those fibers as energy sources."
Research shows the microbiome's composition can change relatively quickly with significantly increased fiber intake, and even modest additions — a handful of nuts or an avocado — can enrich certain beneficial bacteria over a few weeks. Fermented foods such as kimchi, kefir and sauerkraut also show particular promise. In one Stanford study, adults who added extra fermented food over the course of two and a half months had increased microbial diversity and a decrease in markers of inflammation. "It does appear that in the process of fermentation, certain chemicals are made, like lactic acid, that may actually have beneficial effects on the gut," Khoruts said.
Lifestyle and the Bigger Picture
Researchers caution against demonizing any single food, emphasizing that the sum of what a person eats matters more than any one item. Holscher advises that most of the diet should consist of whole foods, minimizing foods high in refined sugars and saturated fats, while leaving room for the occasional treat. There is also growing evidence linking ultra-processed foods to gut diseases and microbiome changes, with studies — primarily in animals — signaling that artificial sweeteners, food additives and emulsifiers can be harmful, though the science remains unsettled. Holscher also recommends limiting alcohol, as heavy drinking can disrupt the microbiome and damage the gut lining.
Lifestyle factors such as sleep, exercise and psychological well-being can meaningfully alter the gut microbiome for better or worse. Khoruts says people with gut troubles like bloating, cramps and constipation — assuming no diagnosed condition — can expect significant improvement from relatively simple lifestyle changes. In Shanahan's view, the research has simply caught up to commonsense advice: "the truth of the matter is more nuanced," and the modern microbiome, while suboptimal in some respects, "may be the best we have because our microbiome has changed and adapted to our environment."
