Novel IL-2 Fusion Approach Targets Root Cause
HCW9302 represents a breakthrough approach to treating alopecia areata (搜索) by targeting the underlying immune dysfunction rather than just managing symptoms. The therapy is a subcutaneously injectable IL-2 fusion molecule constructed using the company's TOBI™ platform technology. IL-2, the active component, maintains proper numbers and functions of regulatory T (Treg) cells that control excessive inflammation caused by other immune cells.
"The goal of this initial trial is to establish the safe recommended Phase 2 dose of HCW9302 that effectively increases Treg cell activity in patients," said Dr. Hing C. The scientific foundation for this approach was recently validated by the 2025 Nobel Prize in Physiology or Medicine, awarded to Drs. Mary E. Brunkow, Fred Ramsdell and Shimon Sakaguchi for discovering that Treg cells (搜索) function as the immune system's security guards, preventing immune cells from attacking the body's own tissues.
Addressing Significant Unmet Medical Need
Alopecia areata (搜索) affects approximately 160 million people worldwide and 7 million people in the United States, representing about 2% of the global population at some point in their lifetime. The condition primarily affects individuals under age 30 and occurs at similar rates in both males and females. Currently, no curative FDA-approved treatments exist for this indication.
"While not life-threatening, alopecia areata (搜索) has no cure. For those who suffer from the burden of this disease, it can severely impact a person's quality of life and self-esteem, leading to increased rates of anxiety and depression," Dr. Wong explained. "Existing off-label treatments may provide some relief of symptoms, but there are often dangerous side effects."
The autoimmune condition occurs when the immune system mistakenly attacks hair follicles, leading to hair loss in localized areas, the entire scalp, or in some cases, the whole body. Patients often experience recurring episodes throughout their lives, with existing treatments like corticosteroids, immunotherapy, Janus kinase inhibitors, and topical solutions focusing primarily on symptom management rather than providing consistent, long-term hair regrowth.
Phase 1 Trial Design and Objectives
The multi-center dose-escalation study is designed to treat up to 30 patients with alopecia areata (搜索). Primary objectives include evaluating the safety of subcutaneous HCW9302 administration and determining the recommended dose level for later-phase clinical studies. Secondary objectives encompass assessment of disease responses and the effects of HCW9302 on proliferation and function of immune cells, particularly Treg cells (搜索).
HCW9302 has demonstrated good tolerability in non-human primate studies, representing what the company considers an important improvement compared to conventional IL-2 therapies. The company believes HCW9302 can suppress the hair-follicle killing activities of auto-reactive immune cells by activating and expanding Treg cells (搜索) while minimizing the risk of broad immunosuppression or unwanted side effects.
Broader Development Pipeline
Beyond alopecia areata (搜索), HCW Biologics plans to explore HCW9302's potential in various inflammatory conditions, including graft versus host disease (搜索) and neurodegenerative diseases such as Alzheimer's disease (搜索), where the therapy has shown activity in relevant animal models. The company has constructed over 50 molecules using its TRBC platform, with three TRBC-based molecules currently undergoing IND-enabling processes for clinical development programs in cancer and age-related diseases.
